Job Description Job Title: Registered Nurse Practitioner (RNP) Department: Medical Reports To: Chief Medical Officer FLSA Status: Full-time, Exempt Summary The candidate is an experienced nurse, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, Georgia State Medical Board requirements for advanced practice nurses must be met. To provide comprehensive primary health care to the patients of the School Based Health Centers and the Community Health Center. Areas of Responsibility Patient Care: • Provide the choice of direct access to an NP, both in the Practice and over the telephone, for the general practice population, • Make professionally autonomous decisions for which he/she is accountable, • Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan, • Initiate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports for the purpose of patient care, • As an independent prescriber or when prescribed under MD: to prescribe safe, effective and appropriate medication as defined by current legislative framework, • Provide safe, evidence-based, cost-effective, individualized patient care, • Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care, • Promote health and well-being using health promotion, health education, screening and therapeutic communication skills, • Refer patients directly to other services/agencies as appropriate, • Take the Practice lead in developing and delivering specialist primary care services, such as long-term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease and hypertension), • Work with nursing, medical and health care assistant colleagues to ensure that Evidence Based Care (e.g. coronary heart disease/Older People/ Diabetes /Mental Health) are being delivered, • Work with multi-disciplinary team within the Practice, and across the wider primary care spectrum to promote integrated and seamless pathways of care, • Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery, • Participate in identification of community health needs and develop patient/family-centered strategies to address them, • Facilitates training of teachers and center personnel on relevant health topics, • Helps to provide training materials, posters, bulletin boards for use in health centers, for staff and use in schools., • Training and Teaching Others - Identifying the educational needs of others, developing formal educational or training programs or classes, and teaching or instructing others to include medical students, residents, fellows, and other medical personnel at multiple levels of training, • Provides performance evaluations and feedback both written and verbal for learners, • Participate in Community and Academic continuing education meetings and work groups, • Performing Administrative Activities - Performing day-to-day administrative tasks such as maintaining information files and processing paperwork, • Plan, implement, or administer health programs or standards within organization, schools or community for prevention or treatment of injury or illness, • Provide Consultation and Advice to Others - providing guidance and expert advice to management or other groups on technical, systems- or process-related topics as well as community outreach, • Participate in the advancement of EHR implementation and utilization, • Participate in grant writing, grant reporting, research and publications related to primary care, school-based health, public health and other similar related topics, • Participates in evaluation and feedback of staff, • Coaching and developing Others - Identifying the developmental needs of others and coaching, mentoring, or otherwise helping others to improve their knowledge or skills, • Active participant in the Quality Assurance/Quality Improvement Committee and Clinical Risk Management Committee and Activities, • Works as a team member with the Emergency Management and Disaster Preparedness component of the Health Centers, • Adhere to all Whitefoord policies and procedures, • RNP unrestricted license in state of Georgia, • Board Certified in Specialty Area- Pediatrics and Adult or Family, • DEA license approved with full prescribing privileges per Federal and State Law, preferred, • Minimum of 4 years outpatient experience, • Pediatric and Adult care experience are a necessity. The successful candidate should have excellent communication skills and be able to quickly adapt and stratify content for a varied age range. This candidate should have a working knowledge of health disparities and underserved community health issues. The successful candidate will include a comprehensive appreciation of cultural awareness and sensitivity within their scope of services., • Act as a team leader and mentor for the nursing team and through this, support their personal development., • Encourage and develop teamwork within practice., • Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff., • Help the practice operate in a cost-effective manner., • Identify and manage nursing care risks on a continuing basis., • Participate in practice meetings and practice management meetings, reporting progress as required., • Participate in audits and inspections as appropriate., • Work closely with the doctors and administrative managers in the setting up and/or improving practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets. REASONING ABILITY: Must be able to exercise sound judgment and concrete thinking when on the spot decision making is required. Ability to seek supervision for emergencies and unusual circumstances.
