Sun River Health is seeking a full-time Clinical Program Director for the A31 position at Westchester Square. Position Summary The A31 Clinical Program Director provides leadership and acts as a role model for all personnel in the planning and delivery of client services. He/she directs program operations and collaborates with all departments to develop, deliver, and evaluate the clinical program services provided to the Sun River Health clients. The Program Director directly supervises all administrative and case management staff and collaboratively supervises all facilities and plant operations at the program site. This position ensures that concrete services are provided to clients in accordance with New York State Office of Mental Health requirements as well as agency policies, procedures, and quality standards. Essential Duties and Responsibilities • Performs other duties as required by the Vice President or his/her designee, • Provide counseling and crisis intervention services as needed., • Recruit, supervise, develop, and evaluate staff, • Responsible for all statistical and other reports as requested., • Responsible for employee time and attendance requests, • Conducts performance management activities, including but not limited to appraisals, documentation, coaching and disciplinary actions for clinical staff, • Oversees all client intakes and discharges and assures that they are processed in accordance with agency and regulatory requirements, • Provides counseling and crisis intervention services as needed., • Adhere to all HIV/AIDS and substance abuse confidentiality requirements., • Analyze program performance and forecasts future demand and resource requirements for continued effective program operation., • Assure compliance with all policies, regulations and laws governing clients' rights and confidentiality of information., • Attend and participates in continuing education programs, seminars designed to maintain professional competence., • Build, develop, and maintain community relationships and referral arrangements with community agencies and hospitals, • Trains all staff on meeting the regulatory standards per OMH, • Screens and selects clinical candidates for program staff positions, • Provides clinical supervision and in-service training for clinical staff, • Coordinates and oversees clinical program and services, • Conduct performance management activities, including but not limited to appraisals, documentation, coaching and disciplinary actions for clinical and administrative staff at the program site., • Conduct quality assurance reviews to monitor and audit compliance with indicators and program metrics. Job Description JOB DEFINITION: This role is focused on the overall administrative coordination of all reportable incidents, accidents and grievances that occur within the organization. It is focused on timeliness and accuracy of reports, generating and tracking investigative reports and corrective action plans, and generating trend reports that focus the management team on organizational risks in this area. EDUCATIONAL REQUIREMENTS: Bachelor's Degree required; Master's Degree in Social Work preferred EXPERIENCE REQUIRED: 3 years direct experience with identifying and processing reportable incidents within a regulatory framework, preferably Justice Center/OMH/OASAS/DOH and OPWDD Incident reporting regulations. DUTIES AND RESPONSIBILITIES: • Initiate reports through the designated organizational process, troubleshooting any issues the team is having with reporting and following through to closure of report in all required systems within the required timeframe, • Adjust report templates and relevant documentation as needed to accommodate recommendations, changes in regulatory requirements and requirements for data analysis, • Logging, tracking and trending of reports including providing a monthly, quarterly and yearly trend report under the supervision of the Compliance Officer. Analyze incident data to identify patterns and emerging risks., • Coordinating updates and communication with all relevant departments about reporting expectations and regulatory updates, • Managing the filing and maintenance of all completed reports, minutes and relevant documents., • Preparing monthly documents for review at the committee or for audit, • Schedule routine and special review meetings, write formal meeting minutes, compile monthly Board report, • Review all communications from regulatory bodies related to requests for revision of reports, collect all necessary documentation to ensure closure of incidents and submit under the direction of the Compliance Officer, • Train staff how to use NIMRs and set up/assist staff with NIMRS access, learn utilize other incident management systems that are updated or newly implemented, • Assist with completing investigation related tasks as assigned by the Compliance Officer including investigative reports, interviews, note taking, generate reports, gather and review documents, and perform on-site assessments., • Issue CAPs for investigations and ensure they are properly circulated and finalized., • File completed investigative reports and closing in NIMRS, the WSIR and internal files., • Assist programs with monitoring of open incidents in NIMRS and ensuring closeout, • Attend all regulatory updates and ensure they are shared with all relevant staff., • Generate training materials and providing training/presentations, guidance and support to all staff in understanding and following the Incident Management guidelines and policies that apply within their programs., • Work collaboratively with the training department to ensure the Incident Management Curriculum is updated on a regular basis., • Ensure Safety Checks are entered promptly into the NIMRS System following any event, • Participates in regular supervision with the Compliance Officer who provides governing oversight for investigations and provide investigative updates, including written notification and follow-up reports, as needed for administrative action, • Must have flexible hours and ability to work remotely and on site as needed. The Office and Field Coordinator will support program operations, scheduling, and facility visits. The Office and Field Coordinator will be responsible for high-volume data entry, monthly reports, answering phones, continuing education tracking, planning events, and conducting outreach in addition to visiting facilities as necessary for advocacy. Minimum of 3 years of experience working with older adults and disabled individuals is required. Understanding of aging, health care, and long-term care issues is preferred. Computer proficiency, including Microsoft Office, is required. Strong interpersonal skills and the ability to deal effectively with a diverse range of people is required. Excellent organizational skills and ability to manage multiple tasks and meet deadlines are required. Excellent verbal and written communication skills, as well as presentation skills, are required.
