Job Description We are seeking a compassionate, reliable, and experienced Direct Care Worker to provide high-quality, person-centered care to clients in their homes. This role involves assisting clients with Activities of Daily Living (ADLs), meal preparation, medication reminders, and safe transfers using a Hoyer lift. Key Responsibilities: * Assist clients with Activities of Daily Living (ADLs) including bathing, grooming, toileting, dressing, and mobility. * Prepare and serve meals according to client’s dietary needs and preferences. * Encourage and remind clients to take prescribed medications as scheduled (Note: No medication administration). * Use a Hoyer lift safely and correctly to transfer clients as needed. * Perform light housekeeping duties to maintain a clean and safe environment. * Monitor and document client’s condition, behavior, and physical/mental status. * Communicate effectively with family members, care managers, and other care team members. * Follow the client’s personalized care plan and agency policies at all times. * Maintain client dignity, privacy, and respect at all times. Qualifications: * Minimum 1 year of experience in a similar home care or direct care role. * Proficient in using a Hoyer lift and trained in safe client transfers. * Knowledge of proper body mechanics and client safety techniques. * Strong communication and interpersonal skills. * Ability to work independently and manage time effectively. * Must be dependable, trustworthy, and compassionate. * High school diploma or equivalent preferred. * Current CPR/First Aid certification (or willingness to obtain). * Background check and references required. Preferred Qualifications: * Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA). * Experience working with elderly or disabled individuals in a home setting.
