Job DescriptionThe Outreach Consultant will be responsible for promoting key aspects of Spence-Chapin's Domestic Adoption Programs. Responsibilities include providing education about S-C's Domestic Adoption Program and Birth Parent Services in New York City, support in implementing outreach strategies to recruit prospective birth parent clients. Individuals and families receive services that will help overcome problems with public benefits, housing, education, health care and other issues that could impede their ability to maintain stable households. Document client progress via detailed progress notes., • Evaluate and recommend closing of cases in which clients: (a) have achieved stability, (b) have not demonstrated a willingness to participate (lost-to-service), or (c) have reached the allotted time of services 120 EAF/90 ESG without recertification or has become ineligible for services (e.g. exceed income guidelines)., • Meet daily, weekly, and monthly goals and tasks as assigned and in accordance with the funder guidelines., • Complete and submit subsidy applications and requests for financial assistance., • Provide all required information for weekly, monthly, quarterly, and annual reports., • Act as client liaison/client advocate with outside organizations regarding matters such as education, healthcare, housing, legal issues, etc., • Assist clients in completing applications for benefits and entitlements, and process applications on clients' behalf (e.g., Access HRA, Housing Connect, DRIE, SCRIE)., • Escort clients to appointments (e.g., social services, healthcare, etc.) as needed., • Other duties as assigned. RESPONSIBILITIES • Assist with the outreach and engagement of residents of 2605 Sedgwick and Bronx Park Phases., • Assist with NYSERDA Green Energy program- program participant and follow-up services., • Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up, • Manage a case load, • Monitor and document client progress toward service plan goals, • Document all interactions with, or on behalf of, clients, • Maintain familiarity with program resources available on- and off-site, • Coordinate client services with other social service, mental health, medical, employment, educational, childcare and other providers, • Complete program reports on a monthly basis, or more frequently as required, • Act as program contact person for inquiries concerning clients, • Provide collaboration and support with securing benefits, especially as it relates to access to benefits services and eviction prevention efforts, • Report to, and meet with, supervisor on a monthly basis, • Conduct home visits and other field work, as required, • Coordinate and facilitate monthly activities that fit the needs of the site, • Perform additional administrative duties as assigned, • Assist with community events as needed and serve as a liaison with community vendors QUALIFICATIONS • Bachelor’s degree with case management experience or High School Diploma with 3 years of case management experience, • Experience using Access HRA and securing HRA entitlements and benefits preferred, • Mental health background preferred, • Bilingual preferred, • Crisis prevention and mental health first aid training is required within first 6 month, • Strong oral and written communication, time management and organizational skills are necessary., • Proficiency in Microsoft Office suite and other standard business technology is required. PHYSICAL REQUIREMENTS • Ability to use a computer for prolonged periods., • Ability to occasionally lift and/or move up to 10 pounds., • Ability to stand, walk, or sit for long periods of time, or climb five flights of stairs, if required to conduct home visits and/or fieldwork., • Ability to bend and retrieve objects and/or documents., • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation., • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
