Job DescriptionThe Outreach Consultant will be responsible for promoting key aspects of Spence-Chapin's Domestic Adoption Programs. Responsibilities include providing education about S-C's Domestic Adoption Program and Birth Parent Services in New York City, support in implementing outreach strategies to recruit prospective birth parent clients. RESPONSIBILITIES • Assist with the outreach and engagement of community members in the E Robert Moore and Courtlandt PACT-RAD development, • Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up, • Manage a case load, • Monitor and document client progress toward service plan goals, • Document all interactions with, or on behalf of, clients, • Maintain familiarity with program resources available on- and off-site, • Coordinate client services with other social service, mental health, medical, employment, educational, childcare and other providers, • Complete program reports on a monthly basis, or more frequently as required, • Act as program contact person for inquiries concerning clients, • Provide collaboration and support with securing benefits, especially as it relates to access to benefits services and eviction prevention efforts, • Report to, and meet with, supervisor on a monthly basis, • Conduct home visits and other field work, as required, • Coordinate and facilitate monthly activities that fit the needs of the PACT-RAD community, • Perform additional administrative duties as assigned, • Assist with community events as needed and serve as a liaison with community vendors QUALIFICATIONS • Bachelor’s degree with case management experience or High School Diploma with 2 years of case management experience, • Experience using Access HRA and securing HRA entitlements and benefits preferred, • Mental health background preferred, • Bilingual preferred, • Crisis prevention and mental health first aid training is required within first 6 month, • Strong oral and written communication, time management and organizational skills are necessary., • Proficiency in Microsoft Office suite and other standard business technology is required. PHYSICAL REQUIREMENTS • Ability to use a computer for prolonged periods, • Ability to occasionally lift and/or move up to 10 pounds, • Ability to stand, walk, or sit for long periods of time, • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork, • Ability to bend and retrieve objects and/or documents, • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation, • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work BronxWorks seeks to build an inclusive organization grounded in respect for differences.
