Job Description Job Title/Position: Admissions Registered Nurse Classification: Non-exempt Reports To: ED/Administrator Job Description Summary The Admissions Registered Nurse (RN) is responsible for conducting comprehensive hospice admission assessments and coordinating the initiation of hospice services for newly referred patients. This role focuses exclusively on the admission process, including gathering clinical information, educating patients and families regarding hospice services, initiating the plan of care, and collaborating with the interdisciplinary team to ensure a smooth and compassionate transition into hospice care. The Admissions RN does not maintain an ongoing patient caseload but instead supports the timely and effective onboarding of patients onto hospice services. Essential Job Functions/Responsibilities Admissions & Clinical Assessment • Conducts admission visits in patient homes, hospitals, assisted living communities, skilled nursing facilities, or other care settings., • Performs comprehensive physical, psychosocial, and environmental assessments of patients referred to hospice., • Reviews medical history, medications, and current clinical status to confirm hospice eligibility criteria., • Accurately documents assessment findings, vital signs, and clinical observations in the electronic medical record in accordance with regulatory and agency standards. Care Coordination & Documentation • Initiates the individualized hospice plan of care in collaboration with the attending physician and interdisciplinary team., • Educates patients and families on hospice philosophy and goals of care, services provided by the hospice team, medication management and roles of interdisciplinary team members., • Obtains and ensures completion of all required admission consents and documentation., • Coordinates services with pharmacy, durable medical equipment (DME) providers, and internal clinical scheduling staff to ensure timely initiation of services. Communication & Team Support • Communicates admission findings, patient needs, and any clinical concerns to the RN Case Manager and interdisciplinary team., • Participates in team meetings, case conferences, and quality improvement activities as needed., • Represents the hospice agency professionally when interacting with patients, families, facilities, and referral partners. Caseload Flexibility • While the primary responsibility of the Admissions Registered Nurse is to conduct hospice admission assessments and coordinate the start of care, the RN may be asked to temporarily assume a patient caseload or provide case management support based on agency needs, staffing levels, or patient care demands., • When assigned a caseload, the RN will perform duties consistent with the RN Case Manager role, including conducting routine and PRN patient visits, managing and updating the patient’s plan of care, coordinating care with the interdisciplinary team, communicating with physicians, facilities, and family members regarding patient status, and completing required clinical documentation in accordance with agency policies and regulatory standards., • The RN will maintain communication with leadership and the interdisciplinary team to ensure continuity of care, appropriate workload management, and delivery of quality hospice services. Position Qualifications • Registered Nurse (RN) with current licensure to practice professional nursing in the State of Oregon., • Minimum of seven (7) years of clinical nursing experience required., • Experience in hospice, home health, skilled nursing, oncology, palliative care, or geriatric nursing preferred., • Current CPR certification required., • Valid driver’s license and reliable transportation required., • Strong clinical assessment, communication, and documentation skills., • Ability to work independently in the field and effectively represent the agency in patient homes and healthcare facilities. Working Conditions • Performs duties in a variety of settings including patient homes, hospitals, assisted living facilities, skilled nursing facilities, and other community environments., • Requires frequent travel within the service area to conduct admission visits., • Works both independently in the field and collaboratively with the hospice interdisciplinary team., • May encounter patients with advanced illness, infectious diseases, and end-of-life conditions, requiring adherence to infection control and safety protocols., • Must be able to adapt to varying home environments and patient care situations., • May require flexibility in scheduling to meet patient admission needs, including occasional evening or weekend visits. Physical Requirements • Ability to stand, walk, bend, stoop, kneel, and reach while performing patient assessments and care activities., • Ability to lift, move, or assist patients up to 50 pounds, with or without reasonable accommodation., • Ability to sit for extended periods while completing documentation and driving between patient visits., • Ability to use hands and fingers to operate medical equipment, electronic devices, and documentation systems., • Ability to communicate effectively with patients, families, and healthcare professionals both verbally and in writing., • Ability to see, hear, and assess patient conditions to safely perform clinical evaluations and provide appropriate care.
