Scope of Duties: • Recruit clients to participate in program including visiting the county jail to interview prospective clients., • Complete intake interviews with clients and secure program participation agreements., • Assist clients apply for social entitlements such as GA, TANF, etc. when appropriate., • Link homeless clients to housing., • Monitor and document client progress., • Assess client needs and work with client to develop goals., • Update service plan with client input., • Provide motivational support to clients in program and maintain support throughout participation., • Schedule and facilitate meetings with clients as required., • Identify and/or develop resources and link clients to needed services., • Facilitate scheduled workshops/programs/groups addressing client's needs., • Develop appropriate discharge plan on all assigned clients., • Participate in required internal/external meetings., • Complete all required reports, data collection and other documentation., • Interact with all clients, staff, and outside contacts with respect and courtesy., • Maintain client and staff confidentiality in compliance with administrative policies and procedures., • Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions., • Occasionally required to smell, stoop., • Regularly required to use hands to handle or feel., • Regularly required to reach., • Regularly required to stand, walk, sit, talk and hear., • Regularly required to lift and move up to 10 lbs., • Ability to see clearly at 20 inches or less., • Ability to observe an area that can be seen up and down or to the left and right while remain fixed on a given point.
