SUMMARY Under general supervision, is responsible for devoting their attention to the care and development of the people we support in one of the Agency’s residential facilities. Interacts in a manner which fosters dignity and learning in a way that helps lead to greater independence. Helps create a homelike atmosphere for the people we support in an agency facility. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Residence Program Specialist will include but are not limited to the following: • Provides care and assists with personal development and independence of the people we support., • Ensures that all measures for safety and well-being are maintained at highest level., • Provides training for the people we support in activities of daily living and in development of self-help and social skills., • Maintains a personalized, homelike atmosphere in the residential facility., • Attends weekly staff meetings as well as any other assigned meetings and in-service trainings., • Prepares concise written reports including Incident Reports, Progress Notes and other reports as assigned., • Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action., • Ensures actions are conducted in a manner that prevents abuse, and reports observations of co-workers who may be abusive in accordance with established procedures., • Administers appropriate first aid based on visible symptoms and medical history of the people we support in accordance with established guidelines and accepted first aid practices., • Summons police, fire, ambulance or other emergency personnel if situation warrants it., • If an emergency has caused a person we support to exhibit behavior that is threatening or dangerous, may have to utilize strategies for crisis intervention in accordance with OPWDD approved Behavior Management guidelines., • Provides transportation for the people we support in agency vehicles to and from work, shopping, programs, restaurants, movies, doctor’s appointments, etc., • Assists with meal planning and preparation as well as with purchasing of food and supplies., • Demonstrates an understanding of and implements behavioral programs., • Participates fully as a member of interdisciplinary plan., • Maintains an ongoing informal exchange of information. Job Description The Assistant Residence Manager is responsible for supporting the Residence Manager in the supervision of individuals’ supports, maintenance of assigned programs, and implementation of Life plans. She/he supports and assists people supported in attaining their needs, desires, and dreams through a person-centered planning focus. She/he assures quality services by providing an environment that is conducive to their welfare of the people supported and in accordance with federal, state, local, and organization regulations and policies. She/he provides support to the Residence Manager in leadership of the team and serves as an advocate to ensure appropriate treatment, teaching, and protection of the person supported rights. She/he develops and cultivates positive relationships with families and other stakeholders. She/he monitors the coordination of individual appointments and activities, and the management of the persons supported personal fund accounts. The Assistant Residence Manager is responsible for the supporting the Residence Manager by working collaboratively with the Quality Improvement team to ensure the best outcomes for the program and individuals, ensures compliance with relevant regulations, and ensures that programs and services to individuals supported are person-centered and directed. The Assistant Residence Manager is an effective communicator. She/he is able to provide person supported and staff constructive feedback regarding performance and behavior. The Assistant Residence Manager is responsible for updating the Residence Manager on a daily basis of all events in the residence. Essential Functions Staffing In partnership with the Residence Manager: • Ensures that the program is adequately and appropriately staffed and monitored., • Ensures the appropriate training of staff who are assigned to the Residence., • Provides weekly supervision meeting for residential staff., • Observes, monitors, and regularly provides feedback to staff regarding the implementation of the individuals’ plans through the use of meal observations, community meetings, and daily performance of goals., • Demonstrates and models’ effective methods to teach existing positive and replacement behaviors to individuals. In partnership with the Residence Manager: • Ensures that the program meets the guidelines and regulations which govern residential operations., • Tracks program and individual progress to ensure further program development., • Maintain and update medication records of individuals residing in the IRA., • Monitor to ensure all medical appointments and follow-up visits are done as needed., • Work closely with Nurse/Medical Director on all medical care for residents., • Work closely with Psychologist on all behavioral plans and data for residents., • Establish professional relationships with local pharmacies, urgent care centers, etc., • Ensure that all reports of injuries and accidents are reported as required by organization policy and OPWDD regulations. In partnership with the Residence Manager: • Monitors staff’s required charting and documentation records and completes reports on person supported progress towards meeting goals and outcome as needed., • Responds to all correspondence promptly and professionally, meeting required timeframes., • Conducts one unannounced visit per month to I/DD IRAs or Day Programs; completing the unannounced visit checklist during each visit and reporting results to Director I/DD and Director QI within one week of visit; and following up on action items at next unannounced visit., • Ensures the proper documentation and reporting of all incidents and events to all of the proper Organization and Monitoring authorities., • Ensures the review of all incidents and events on a monthly and quarterly basis., • Compiles other reports and data for management review., • Ensures that all reports of injuries and untoward incidents for staff and individuals are reported appropriately., • Ensures that appropriate training and documentation of training for individuals and staff is up to date and meets standards for reimbursement as well as program guidelines., • Ensures that all billing information for the program is completed and submitted to accounting in a timely manner., • View Foothold records on a daily basis to ensure that individuals’ goals are recorded., • The Assistant Residence Manager is responsible for the supporting the Residence Manager in monitoring and reviewing all documentation in accordance with federal, state, local, and organization regulations and policies, and ensuring all documentation is done in a timely manner. Job Path’s Supported Living program strives to give adults with disabilities to live in a home that is their own. The apartments we provide support in are warm and inviting, with one to three people sharing a home. Each person has their own bedroom and is empowered to make choices about their lives. We are currently seeking a Coordinator in our Supported Living program who will lead a team of Direct Support Professionals to provide support that centers the needs, expectations and individual differences of the people we support, acting with care, dignity, and compassion. A strong candidate for this role will bring strong leadership skills, with an emphasis on critical thinking, clear communication, and follow through. Be sensitive to the needs, expectations and individual differences of the people we support, acting with care, dignity, and compassion. Job Path operates under OPWDD and thus must ensure compliance with their regulations. This is a full-time position at 35-40 hours per week, those hours may include some evenings and weekends. A team of six rotates evening and weekend on call responsibilities. Responsibilities of the Coordinator in Supported Living: Centering the People we Support • Foster open and supportive communication between the people we support, families, and other community organizations and agencies., • Coordinate/attend meetings related to the people we support (Life Plans, Residential Plan reviews), ensure that next steps are appropriately delegated or followed through on, • Implement and follow-up on all support services recommendations., • Act as liaison for involvement in support groups, self-advocacy, vocational programming, transportation services, integrated community activities, recreation, and learning independent living skills., • Work closely with the Medical Coordinator and the Registered Nurse to review medical documentation and ensure all medical follow ups are being done in a timely manner., • Create and distribute the weekly staffing schedule, ensuring that all activities are included and that appropriate levels of coverage are maintained. This includes providing coverage as needed., • Schedule formal supervision time with staff on a regular basis, at least monthly, to cultivate a working relationship where team members can discuss challenges and opportunities that they are facing, and constructive feedback can be delivered and received., • Train staff and support their development through teaching, acting as a role model, and setting up shadowing shifts with strong DSPs. This includes guiding staff on how to appropriately work with and interact with the people we support, other staff, families and guardians., • Complete annual evaluations for staff and work with them to review plans for professional development in partnership with the Human Resources team, • Follow through with ensuring that for Direct Support Professionals complete required trainings, in partnership with the Training and DEIAB Coordinator, • Participate in interviews to screen new staff, • Schedule and conduct fire and emergency drills with staff and people who we support, making sure to log the drills, • Possess a strong working knowledge of all required emergency procedures in order to provide leadership during practice and actual drills., • Maintain and track inventory of PPE in each apartment, • Create schedule blocks in the electronic health records system, being mindful to maximize opportunities to bill appropriate programs, • Review and approve casenotes submitted by staff and follow up to ensure timely completion, • Ensure completion/submission of all reports as scheduled. This includes directing staff, providing consumers with proper care, ensuring the safety of the consumers, scheduling necessary repairs and the general upkeep of the facility. Administers designated program in accordance with Agency policies and regulations and requirements of applicable regulatory agencies. Is responsible for all aspects of program including personnel, budgeting, services to individuals and physical plant management. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Director of Residential Program Services – Residential Services will include, but are not limited to the following: • Works closely with Interdisciplinary Clinical team to coordinate residents’ treatment plans., • Ensures that all measures for safety and well being are maintained at highest level., • Provides overall supervision to residence staff including Registered Nurse/Licensed Practical Nurse, Residence Manager, Senior Residence Program Specialist, Case Manager, Residence Program Specialist, Recreation Specialist utilizing flexible time scheduling to make on-site consultations and supervision of each shift., • Conducts weekly/biweekly staff meetings with all direct care staff., • Interviews, evaluates, hires, orients, trains, disciplines and when necessary, terminates subordinate staff., • Maintains and posts scheduled work assignments and adjusts such assignments based on availability of staff, individual abilities of staff and particular requirements of the people we support., • Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action in absence of a nurse., • Ensures actions are conducted in a manner that prevents abuse., • Administers appropriate first aid based on visible symptoms and resident history in accordance with established guidelines and accepted first aid practices., • Summons police, fire, ambulance or other emergency personnel if situation warrants it., • Maintains records on subordinate staff such as level of performance, specific deficiencies, counseling sessions training records, etc., • Conducts weekly individual supervisory meetings with subordinates in order to discuss program, personnel or training needs and assesses their performance., • Responds to program and work-related questions, problems, complaints and grievances from staff., • Develops residence facility rules and procedures., • Ensures residence facility is kept clean, neat, healthy and safe., • Assures that all staff are coordinating their efforts in order to complete such routine household tasks as laundry, menu preparation, shopping, cooking, etc. with goal of increasing independence of residents in performing these functions., • Directly responsible for all medical issues of residents., • Works with appropriate clinical or program staff and community agency staff to identify and maintain placement in day programs, workshops, school programs, etc. which are in community and are required in residents’ service plan., • Acts on behalf of residents at schools or school boards, health clinics, local businesses, employers, transportations, providers, social groups, neighbors, etc. in order to ensure fair and appropriate consideration.
