Job Summary As a collaborative member of the Respite team, this position is responsible for assisting clients in the House of Charitys Respite program by increasing client social determinants of health through education and opportunities to engage in activities that support client successful transition from Respite into independent living. Job Duties/Responsibilities To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below. Reasonable accommodation may be provided to enable individuals with disabilities to perform the essential duties/responsibilities of this job. * Coordinate intakes with local hospitals, medical clinics, and other providers by receiving and reviewing referrals and preparing beds for all incoming Respite clients, in accordance with current procedure. • Enter daily attendance and complete and accurate service notes in the required databases. Complete intakes, enrollments, discharge plans, and exits in accordance with WAC, funder, and internal requirements. * Help facilitate client connection to internal resources as needed, such as Behavioral Health Team and Case Management. Maintain a current knowledge of community resources available to clients. * Assist respite clients in independent living skill-building, including financial and life-skills coaching. * Collaborate with the Respite Peer Specialist to coordinate activities and communitybuilding opportunities for Respite clients for healthy community engagement. * Maintain client files according to funder requirements. Ensure client data in HMIS is current, accurate and entered in real time. * Regularly monitor and assist in maintaining a clean and safe environment for all clients, staff and volunteers. Assist in ensuring that Respite Rules are being observed. * Upon client exiting program, clean and sanitize bed, sleeping area, and locker. Secure and process belongings left behind by discharged Respite clients. Document this in accordance with current procedure. * Assist in the operation of the Respite Program by coordinating transportation, reminding clients of appointments, and checking daily on the safety and well-being of all Respite clients. Responds to residents complaints. * Support Respite staff in accomplishing daily tasks to support client stability and care. This may include med runs, cleaning the dorm, and making beds when necessary. * Intervene in crises, respond to emergencies, and initiate action as required, including contact with emergency response systems. * Write significant events involving residents and building operations activities in a daily log. Reads log daily. * Performs other relevant duties assigned by supervisor. * Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA). * Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor. * As a mandated reporter, follow all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect. * Adheres to the tenets of Catholic Social Teaching and Catholic Doctrine. * Perform as a team member to ensure that productivity outcome measures are achieved. * Performs related functions necessary to support the mission and core values of Catholic Charities. Job Qualifications To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job. Education/Experience: to perform this job successfully, an individual must have a bachelors degree in social services or related field and one year of relevant experience in direct client services; OR an AA Degree with 3 years of relevant experience in direct client services; OR 5 years of relevant experience in direct client services. Successful candidates will demonstrate in their cover letter how they meet the minimum level of education and/or experience requirements. Certificates/Licenses: To perform this job successfully, an individual must have a valid Drivers License and the ability to drive for work use. Successfully pass background check applicable to position.
