Specifically, you will: • Assess a patient’s condition to determine the severity of the problem., • Speak with patients to elicit their medical history and learn the details of a patient’s ailments., • Perform physical examinations to diagnose and treat patients., • Able to stabilize or attend to the patient’s basic needs and refer patients to a specialist, admit, or transfer the patient as necessary., • Able to quickly assess a patient’s condition with very little information, make a preliminary diagnosis, and immediately begin treating a patient., • Effectively interacts with diverse cultural backgrounds., • Effectively interacts with a wide range of personality types and ages., • Effectively fills out legal record of treatment and for the benefit of other healthcare professionals., • Promote health education., • Liaising with other medical and non-medical staff in the company to ensure quality treatment and working together as a team., • Be available to respond to request for house calls within a reasonable response time. • Board Certified/Board Eligible in Emergency Medicine, • ACLS certified, PALS certified, • Residency trained and board certified in Emergency Medicine by ABEM., • 5 years of emergency room experience preferred., • Experience with point of care ultrasound and diagnostic testing preferred (e.g. biofire)., • Experience with Athena preferred, but not required., • Prior concierge medicine experience preferred, but not required.

Job Description In the role of a Registered Nurse, you will provide concierge emergency medical care to Sollis Health patients in our clinics, with the supervision/oversight of an attending Physician or Physician Assistant. This is a pivotal role as part of a growing organization providing seamless care to both adult and pediatric patients for cases that would usually be treated in a traditional emergency room. Per Diem - 7a-3p, 3p-11p weekdays and weekends Responsibilities • Providing excellent medical care and treatment for patients in their home or wherever they request with the supervision/oversight of an attending Physician or Physician Assistant, • Assessing and treating Covid-related cases which includes obtaining clinical samples to test for the virus, performing point of care testing, obtaining blood for antibody blood tests, providing general symptom management and recommendations, • Performing medical procedures that include obtaining IV access, phlebotomy, administering vaccines, performing POC rapid tests, administering IV medications and fluids, and performing EKGs, • Documenting patient information in the electronic health record, Athena Health and in our CRM Salesforce, • Responsibility for maintaining and tracking medication inventory, • Coordinating additional care with our After Care, and house calls teams or with one of our Medical Partners, • Working closely with our clinical team members to ensure all policies are adhered to and that you deliver quality care in an innovative setting, • Making patients comfortable, patiently and professionally answering any questions they have and using patient-centered empathetic communication at all times, • Graduated from an accredited School of Nursing. Associate's or Bachelor's Degree in Nursing, • Current License to practice as a Registered Professional Nurse in Florida State required, • BLS, ACLS, PALS certification required upon hire, • Strong clinical skills, • Tech savvy with the ability to use and or learn our systems including but not limited to Microsoft Teams, Athena Health, Salesforce and Humanity, etc., • Superb technical skills, including phlebotomy, peripheral IV placement, medication administration, • Ability to be adaptable and work in a fluid, multitasking environment, • Strong process and organizational skills, • The ability to problem-solve and think outside of the box Sollis Health is the first and only medical membership that's on-demand 24/7, 365. Job Description POSITION PURPOSE: Maintains an active caseload of clients who need extensive care management and supportive oversight on a temporary basis. POSITION REQUIREMENTS / QUALIFICATIONS: Education/Experience: Graduate from an accredited college or university with a Bachelor of Science or Arts Degree in the Social Work, Psychology, Human Services, Nursing, Science or Health field. Minimum of six months' experience working in the field of social work or case management. Familiarity with the local community resources. Licensure / Certification: CPR Certificate from the American Heart Association. Valid Florida Driver’s License. Skills / Ability: Verbal and written fluency in English. English required bilingual desirable. POSITION RESPONSIBILITIES (THIS IS A NON-EXEMPT POSITION):  Maintains an active caseload of clients who need case management services and extensive oversight on an ongoing basis.  Implements ongoing monitoring of clients’ needs, and evaluation of services to ensure collaborative care among the Population Health staff, interdisciplinary team members and outside service constituents.  Exercise and promote a functional referral system with all departments within CHI system to ensure quality care.  Network with various external resources to provide services for patients as needed.  Ensure active caseload of patients have an appropriate interdisciplinary staff especially with center nursing team to ensure patients are successfully transfer within 6 weeks no later than 8 weeks for management.  Maintains records of services provided to clients and completes records for funding programs, also assisting with gathering and compiling information for internal and external reports  Participates in patient/community education (internal and external) and home visits when necessary.  Communicates pertinent information to supervisor / appropriate staff.  Participates in Quality Assurance Program and continuing education as required.  Maintains all licenses and/or certificates as required for the job.  The Population Health Care Management Specialist must monitor clients’ adherence to treatment and medication regimens.  Encourage individuals to become involved in consortium activities.  Participate in any other functions to meet the goals of this activity.  The Population Health Care Management Specialist must collaborate with other caregivers (i.e. medical care provider, nutritionist, prescription drug provider, etc.) for adherence and assistance in meeting treatment objectives.  The Population Health Care Management Specialist will also be expected to empower clients to be actively involved in the development and monitoring of their treatment and adherence plans and must ensure that immediate follow-up is available for clients who miss their prescription refills and/or who experience difficulties with adherence.  Maintains and meets monthly productivity standards.  Reports to work on time and ready to work with minimal absenteeism.  Completes B&E (billing and encounter) forms within the same day of intervention.  Provides accurate and timely documentation in patient charts within the same day of intervention.  Adheres to Confidentiality Policies and Procedures / HIPAA Regulations.  Performs other duties as assigned.
