Job Description Position Overview: The Physician works with the Center Leadership Team at assigned Center, and under the direction of the Chief Medical Officer directs the Center's medical policies and programs ensuring the delivery of affordable, efficient, responsible, and accountable occupational healthcare of the highest quality to business, industry and patients in accordance with Agile’s policies, practices and procedures. This position will be shared between two clinic sites: Sacramento and Stockton. The candidate must be able to reliably commute to and work at both locations as scheduled. Position Summary The case manager provides support, education, and resources to facilitate success in the community to adults who are diagnosed with mental illness and are also justice-involved. This includes meeting with clients in correctional facilities, the community, their home, and the office. The case manager completes appropriate assessments, care plans, and progress updates and collaborates with treatment team members and community stakeholders. Required Qualifications • High School diploma or equivalent required., • Two plus years’ experience working with justice-involved adults OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job., • Knowledge and training in educational and employment opportunities in the community, • Strong local network preferred, • Excellent interpersonal skills including active listening, problem solving, and the ability to remain calm in emotional or stressful situations. · Assist and support the Center/Department/Program to meet standards of High Reliability. · Must have valid driver’s license and insurable driving record. Essential Responsibilities • Provide education services at THRIVE that include: case management, substance abuse education, job readiness, employment services, job coaching, life skills (including financial barriers and training), mentorship, recreational activities, and transition plans. · Provide consultation and technical assistance regarding changes or enhancements to program delivery to ensure efficacy. • Other duties as assigned.
