Position Summary The Clinical Quality Coordinator plays an active role in managing and facilitating WellSpace Health’s Quality Plan. Responsibilities include assisting with the development, implementation and evaluation of projects to improve quality metrics; problem solving and developing positive working relationships at all levels. Will manage projects and improvement activities intended to improve UDS and HEDIS rates. Will work directly with the Quality, Medical, and Operational staff, plus IPA’s and Health Plans to identify patients that have gaps in care and to engage patients in seeking appropriate care. Will work directly with the care teams to share gaps in care. Work directly with patients to understand the importance of preventative screenings. The Clinical Quality Coordinator will play an active role in interventions that close care gaps and improve outcomes for patients. Will support population health efforts and strategies. Required Qualifications • Bachelors or Associate degree in Public Health or related field preferred., • Fluency in additional languages preferred., • Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, and/or physiologically impaired., • Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills., • Excellent interpersonal skills., • Strong customer service experience including active listening, problem solving, and the ability to remain calm in emotional or stressful situations., • Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality., • Compliance with all state and federal laws and regulations as they pertain to position including infection control, sexual harassment, Mandated Reporting, Scope of Practice, OSHA, etc., • Assist and support the Center/Department/Program to meet standards of High Reliability., • Ability to work extended hours and/or weekends related to Health Center hours of operation or special events and programs. Essential Responsibilities • Assist the Clinical Quality Manager with public health initiatives and public health grants for the Infectious Disease Sexual Health (IDSH) Program and Infectious Disease Specialty Department., • Assist with organizing community events, telehealth appointments and meetings with community partners., • Maintain work plan, progress reports, and service records for certain projects as designated by the Clinical Quality Manager., • Oversee maintenance of required documents for auditing purposes., • Create and submit periodic progress reports., • Work closely with Clinical Quality team., • Assist with related medical record data collection and data analysis., • Assist with clinical quality projects and initiatives., • Maintain and adhere to confidentiality, and privileged communications (patient, employee, and organization)., • Perform other duties as assigned., • Monitor WSH, HEDIS and UDS quality measures to identify corrective actions as needed and reports on results, • Complete patient-centered outreach calls and schedule preventative screenings and other medical appointments for the patients, • Attend trainings and staff meetings as required and performs related work as assigned, • Maintain and adhere to confidentiality, and privileged communications (patient, employee, and organization), • Perform other duties as assigned Benefits Successful candidate will receive regionally competitive salary, above average health benefits at reduced costs, company paid life insurance & long-term disability insurance, additional voluntary retirement plan with company match and no vesting schedule requirement. • Paid bereavement and jury duty leave, • 11 paid holidays per year, • Paid time off, • Paid sick leave, • Flexible Spending Program, • Company paid malpractice insurance for all providers, • Professional development hours offered annually Physical Demands and Work Environment The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands and arms to reach. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet. The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care.
