Overview This is an excellent opportunity for a Nurse Practitioner to join a collaborative nephrology practice focused on delivering high-quality, patient-centered kidney care. The NP will work closely with nephrologists and interdisciplinary teams to manage patients with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD), emphasizing education, care coordination, and comprehensive disease management. Key Responsibilities Clinical Care & Patient Management • Provide ESKD patient rounding, including limited and comprehensive visits, under the guidance of nephrologists, • Deliver holistic ESKD care management, including:, • Patient activation and engagement, • Depression screening and support, • High-risk hospitalization oversight, • Transitions of care, • Management of co-morbid medical conditions, • Conduct annual comprehensive ESKD disease management visits, • Collaborate with nephrologists, dialysis center interdisciplinary teams, RN care managers, hospital discharge teams, primary care providers, and specialists as clinically indicated, • Perform physical examinations and assess patient health status, • Order, interpret, and evaluate diagnostic tests and laboratory studies, • Develop, implement, and adjust treatment plans as needed, • Prescribe medications in accordance with scope of practice and state regulations, • Document patient encounters accurately within the electronic medical record, • Promote a high standard of patient service and professionalism, • Build effective working relationships across clinical and operational teams, • Participate in team-based care and adhere to organizational policies and procedures Required • Current Nurse Practitioner (Advanced Practice) license in the state of practice, • Appropriate specialty certification, • Strong understanding of physician practice operations, protocols, and workflows, • Prior nephrology and/or dialysis experience, • Medical and pharmacy coverage, • Dental and vision insurance, • Life insurance, • Virtual care services, • Voluntary benefits, • 401(k) with employer match, • Paid Time Off (PTO)
