Position Overview: The Dialysis Nurse will provide specialized nursing care to patients undergoing dialysis treatment. Job Description The Home Care Coordinator is responsible for maintaining ongoing communication with the clinical team, caregivers, patients, and family members. Include communication with referral and intake sources for all patients. The Home Coordinator shall plan for home healthcare service coverage.
