WHAT YOU’LL DO The Adult Health Home Manager is responsible for the overall daily operations of our Adult Health Home Program and staff. The Manager directs the development, coordination, integration, and delivery of Health Home care coordination services by staff that meaningfully engage partners in care. Additionally, the Manager ensures that all required documentation is correctly completed by staff in a timely manner and completed in compliance with Health Home standards. Key Responsibilities • Supervise the day-to-day operations of the program including management of caseload across staff., • Supervise and provide leadership to staff, assuring the delivery of high-quality patient care, including adherence to professional standards of care, licensing, and regulatory requirements., • Effectively communicate job expectations and follow-up by planning, monitoring, and appraising job results., • Ensure files, case notes, and documentation are all in compliance with program requirements., • Facilitate staff meetings and provide on-going staff training., • Ensure that care coordinators complete the monthly High, Medium, Low Assessment for monthly health home billing and assist staff in this task as needed. Review client progress notes to ensure that a billable service has been delivered and documented in the client chart each month., • Monitor staff performance of core service delivery, including monthly core service delivery percentage., • Assist staff with delivering services to clients as needed, including providing caseload coverage during times of vacancy., • Complete all required chart audits in compliance with the lead health home, along with on-going chart reviews and work in collaboration with the Director of Care Coordination on other quality assurance activities., • Conduct hiring and on-boarding activities, including the training and mentoring of new staff., • Ensure that staff working with HARP enrolled patients complete the HCBS workflow and comply with all HARP standards. Completes HCBS Eligibility Assessments as needed to assist staff., • Ensure that Health Home Plus (HH+) clients are appropriately identified and assigned to the HH+ Care Coordinator. Ensure that HH+ services are delivered to clients and documented in the Netsmart chart., • Work in collaboration with the Director of Care Coordination and other agency managers to both increase internal referrals to Health Home and to ensure Health Home clients are utilizing any other needed agency services., • Help identify new funding opportunities and care coordination programs to support adult populations in need., • Bachelor's degree with five (5) years human service experience., • Applicable background and experience relevant to Health Homes., • 1-3 years program management or supervisory experience preferred., • Ability to operate at a high level of independence and responsibility., • Knowledge of the local service provider networks, particularly related to health homes., • Ability to interpret and respond to program and funder guidelines., • Exhibited ability to effectively work within an inclusive and culturally and linguistically diverse environment.
