Completing all case management documentation and reports and maintaining individual youth files. Reviewing medication logs daily. Conducting weekly audit of medication logs to ensure compliance with medication administration policy. Implementing behavior modification plans established by the team to address discipline issues and the consumer’s lack of adherence to program rules, as needed. Prospective applicants must have a Bachelor’s Degree or Master’s Degree in social work or human services related field; one to three years of experience working with young adults and adolescents, particularly at-risk individuals; excellent organizational skills; demonstrated leadership ability and sound judgment; ability to work effectively as a member of a team; ability to work collaboratively with community resources; excellent interpersonal, verbal and written communication skills; and a valid driver’s license.
