The ideal candidate will beself-sufficient, have strong computer skills, superior relationship management skills, fluent in Spanish and have at least one year of experience in support coordination. The duties of this position center on coordinating services for individuals with developmental and intellectual disabilities and advocating for their wants and needs by providing support and guidance. The responsibilities of Joyful Frontiers Support Coordinators include: - Develop and manage Individualized Support Plans (ISP) and Person Center Planning Tools (PCPT) for individuals. - Coordinate services to meet the needs of the individuals. - Work collaboratively and effectively with individuals, families, and service providers to ensure care needs are met. - Provide continuous monitoring and review of ISPs and budgets as needed. - Remain up-to-date with industry standards and regulations regarding disability services. - Maintain individuals’ confidentiality as required by HIPAA Laws and company policy. - Travel to complete monthly visits - Complete monthly calls - Other duties as assigned.
