CABS Health Network is seeking a Director of Care Operations to provide senior operational leadership across its Home Care, Care Management, and Social Care Network programs. Reporting to the CEO, this role is responsible for strengthening service delivery, improving cross-department coordination, ensuring regulatory and contractual compliance, and advancing high-quality, person-centered care for vulnerable populations across New York. The Director of Care Operations will lead day-to-day operational execution, support program growth, oversee performance improvement, and work closely with clinical, finance, technology, and program leaders to ensure that CABS’ services are delivered efficiently, consistently, and in alignment with organizational goals. This role is ideal for a healthcare operations leader with experience managing integrated care programs, community-based services, Medicaid-funded programs, and multidisciplinary teams. Key Responsibilities • Provide operational leadership across Home Care, Care Management, and Social Care Network programs to ensure coordinated, high-quality service delivery., • Translate organizational priorities into clear operating plans, workflows, performance expectations, and accountability structures., • Supervise, coach, and support directors, associate directors, managers, supervisors, and program leads to strengthen execution and team performance., • Improve coordination between clinical, care management, intake, scheduling, social care, and administrative teams to support seamless client experience and transitions of care., • Monitor program performance to ensure services meet contractual, regulatory, quality, productivity, and client outcome expectations., • Ensure operational compliance with applicable federal, state, local, Medicaid Managed Care, DOH, LHCSA, care management, and Social Care Network requirements., • Lead performance improvement initiatives using data, dashboards, audits, staff feedback, client experience information, and quality indicators., • Partner with Finance, HR, IT, and program leadership to align staffing, budgets, technology, workflows, and reporting with program needs., • Oversee operational readiness for new programs, pilots, service lines, contracts, and partnerships aligned with CABS’ mission and growth strategy., • Support the development and implementation of workflows, policies, procedures, training tools, and documentation standards across programs., • Track and strengthen key performance indicators, including census, referrals, engagement, service utilization, documentation timeliness, client outcomes, quality measures, and staff productivity., • Oversee vendor and partner coordination for nutrition, housing, transportation, environmental health, and other social care service lines., • Review operational, quality, compliance, and performance reports for accuracy, completeness, and readiness for leadership, funder, partner, and board review., • Support partnership development with hospitals, managed care organizations, IPAs, government agencies, community-based organizations, and social care partners., • Promote a culture of accountability, collaboration, equity, continuous improvement, and service excellence across all assigned departments., • Identify operational risks, service gaps, staffing challenges, and process breakdowns, and implement corrective action plans to improve performance., • Represent CABS in selected external meetings, workgroups, coalitions, and partner discussions related to care delivery, population health, social care, and healthcare transformation. Qualifications • Master’s degree in Healthcare Administration, Public Health, Nursing, Social Work, Business Administration, Public Administration, or a related field required., • Clinical background strongly preferred, including RN, NP, MSN, or related healthcare credential., • MBA, MPA, MPH, or equivalent graduate-level management training preferred., • Minimum of 7–10 years of progressive leadership experience in healthcare operations, home care, care management, community health, social services, managed care, or integrated care delivery., • Experience overseeing multidisciplinary teams, program operations, quality improvement, compliance, and performance management., • Strong understanding of Medicaid Managed Care, Social Care Networks, home and community-based services, value-based care, and population health preferred., • Demonstrated ability to manage complex workflows, lead teams through change, improve performance, and build operational systems., • Strong communication, relationship management, analytical, problem-solving, and organizational leadership skills. Preferred Experience • Experience working with vulnerable populations, older adults, individuals with chronic conditions, Medicaid members, and communities impacted by social drivers of health., • Experience with LHCSA operations, Health Home/Care Management programs, Social Care Network services, or Medicaid 1115 waiver-related programs., • Familiarity with quality assurance, audit readiness, documentation standards, utilization tracking, and service performance dashboards., • Experience supporting partnerships with hospitals, MCOs, IPAs, government agencies, and community-based organizations. Position Summary for Posting The Director of Care Operations will play a central role in strengthening CABS Health Network’s care delivery infrastructure. This leader will ensure that programs are well-coordinated, staff are supported, clients receive timely and effective services, and the organization is positioned for continued growth across home care, care management, and social care delivery. The role was adapted from the original Executive Director description and reframed to focus on operational leadership, cross-program execution, compliance, quality, and care delivery performance. Salary + Benefits - $180K - $190K
