Job Summary: The Case Manager is responsible for supporting the Program Social Workers in the provision and coordination of casework services for youth of the Runaway Homeless Youth program Job Responsibilities: • Provide casework services to residents of the shelter including referrals for entitlements, housing, and services, accompanying residents on appointments and fostering/facilitating relationships with other services providers to meet the needs of the residents of the program., • Maintain up-to-date case record documentation and reports including progress notes court reports, and all documentation required by city and state governing bodies., • Enter case information into DYCD’s PTS and other demographic Systems. Provide timely communication to DYCD, • Developing, updating, following up on service/treatment plans.
