Position Summary: California Advancing and Innovating Medi-Cal (CalAIM) is a multi-year initiative by the Department of Health Care Services (DHCS) that aims to revolutionize healthcare for all Californians. You will play a pivotal role in this historic change by helping Medi-Cal members navigate the healthcare system and access comprehensive care. You will be at the forefront of creating a more coordinated, person-centered, and equitable health system that goes beyond traditional clinical settings and addresses all physical and mental health needs. This position is non-clinical and plays a critical role in supporting Medi-Cal members experiencing homelessness and complex health challenges through housing-focused Community Supports services. Abrazar is seeking a committed and highly organized Housing Care Manager to help lead CalAIM housing services and strengthen housing stability for Medi-Cal members experiencing homelessness and complex health challenges. This role supports Housing Transition Navigation Services (HTNS), Housing Tenancy and Sustaining Services (HTSS), Housing Deposits, and Transitional Rent referrals while supervising Housing Care Navigators and related program staff. This is an opportunity for a strong, mission-driven leader to help shape a growing housing program, support frontline staff, improve service delivery, and contribute to meaningful housing outcomes across Orange County. The ideal candidate brings strong people-management skills, sound judgment, and the ability to guide teams through program updates, workflow implementation, compliance expectations, and complex member situations. This position reports to the CalAIM Director and serves as a key operational leader within the housing program. The role includes regular field-based work throughout Orange County, including community meetings, member support activities, housing-related coordination, and partner engagement. Position Responsibilities: Program Leadership & Staff SupervisionSupervise, support, and provide ongoing leadership to Housing Care Navigators and related housing program staff. Promote a collaborative, trauma-informed, culturally responsive, and member-centered team environment. Provide day-to-day oversight, coaching, accountability, and performance management to ensure high-quality service delivery and achievement of program goals. Lead regular team meetings, case consultations, workflow reviews, and staff support sessions. Assist with staff onboarding, training, mentoring, and professional development related to CalAIM Housing Trio services and housing navigation. Support staff performance management, coaching, corrective action processes, and professional development planning as needed.Monitor staff productivity, service quality, timelines, and member outcomes to ensure program effectiveness and compliance. Ensure continuity of operations and provide leadership coverage in the absence of the CalAIM Director. Program Operations & ImplementationSupport implementation and ongoing management of CalAIM housing programs, including Housing Community Support services, Transitional Rent referrals, and related housing workflows.Assist with the development, implementation, and continuous improvement of program workflows, policies, procedures, and service delivery standards.Support CES-related coordination, including referral pathways, placement meetings, housing match processes, and alignment with local Coordinated Entry System (CES) requirements.Serve as an HMIS administrator or program-level HMIS lead by supporting user access, data quality, troubleshooting, reporting, documentation standards, and staff training.Ensure staff adherence to DHCS CalAIM requirements, internal policies, CES/HMIS expectations, documentation standards, and program procedures.Maintain up-to-date knowledge of CalAIM requirements, Housing Community Supports, CES processes, HMIS standards, housing resources, and industry best practices.Support operational improvements, quality assurance initiatives, audits, compliance monitoring, and data integrity activities.Assist leadership with reporting requirements, program tracking, data collection, performance metrics, and outcome monitoring.Review program data, housing outcomes, and service metrics to support program performance, reporting accuracy, and continuous quality improvement.Support implementation, tracking, reporting, and ongoing compliance related to housing-focused grants, initiatives, and funding opportunities, including CalAIM Housing and Homelessness Incentive Program (HHIP) activities and deliverables.Assist with monitoring grant objectives, timelines, documentation requirements, and program deliverables to support successful implementation and reporting outcomes.Identify service gaps, operational challenges, data quality concerns, and training needs, and collaborate with leadership to implement effective solutions. Member Support, Escalation & Service OversightProvide guidance, consultation, and oversight to Housing Care Navigators supporting individuals, families, and youth experiencing homelessness.Provide direct member support and maintain a limited caseload to support program operations, staffing coverage, and CalAIM service requirements, while prioritizing leadership, supervision, and program oversight responsibilities.Support staff in resolving complex, high-acuity, escalated, or time-sensitive member situations related to housing access, placement, stabilization, and retention.Review and support housing-related assessments, eligibility determinations, housing plans, and Transitional Rent referrals to ensure accuracy, appropriateness, and compliance.Provide leadership support, direct intervention, and limited temporary member coverage in complex, escalated, urgent, or operationally sensitive situations as needed to support operational continuity and CalAIM service requirements.Assist staff with complex housing search barriers, landlord engagement, move-in coordination, tenancy issues, and housing stabilization strategies. Care Coordination & Community CollaborationCollaborate with housing providers, landlords, property managers, housing authorities, CES partners, community-based organizations, and service providers to coordinate housing-focused supports for members. Work closely with ECM providers, including the internal ECM team, to ensure housing services are aligned with members’ care plans, health needs, and overall care coordination goals. Support development and implementation of person-centered housing plans addressing housing stability, tenancy barriers, benefits, income, medical needs, behavioral health needs, and supportive services. Coordinate across housing, healthcare, behavioral health, ECM, and community service systems to ensure members receive integrated and comprehensive support. Build and maintain strong relationships with landlords, property managers, housing programs, community partners, and service providers to expand housing opportunities and strengthen referral pathways. Advocate for members’ access to safe, affordable, and stable housing, as well as healthcare, behavioral health, benefits, and community-based resources that support housing retention. Facilitate effective communication among housing partners, ECM care teams, service providers, and external stakeholders to support coordinated service delivery and positive member outcomes. Documentation, Compliance & Quality AssuranceEnsure timely, accurate, and compliant documentation of all member interactions, assessments, care plans, referrals, housing activities, service coordination, and outcomes. Maintain strong documentation practices that support program integrity, CalAIM compliance, billing accuracy, audit readiness, and continuity of care. Monitor compliance with DHCS CalAIM requirements, documentation standards, confidentiality requirements, fair housing principles, and internal agency policies. Support case reviews, audits, quality assurance activities, corrective action follow-up, and documentation improvement efforts. Maintain accurate program records, service tracking, HMIS-related documentation, and reporting records as applicable. Stay informed on CalAIM requirements, housing regulations, fair housing practices, community resources, and policy changes impacting housing and healthcare services. Uphold professional, ethical, trauma-informed, culturally responsive, and member-centered service standards during all in-person, virtual, and community-based interactions. REQUIREMENTS Minimum QualificationsBilingual in English and Spanish or English and Vietnamese.Bachelor’s degree in Social Work, Human Services, Public Health, Urban Planning, Public Administration, or a related field. Equivalent combination of education and relevant experience may be considered. Minimum of two (2) years of experience in housing navigation, homelessness services, case management, care coordination, or a related social services role, preferably in Orange County. Minimum of one (1) year of supervisory, leadership, or team lead experience within a nonprofit, healthcare, government, housing, or social services setting. Demonstrated experience working with individuals and families experiencing homelessness and/or complex medical, behavioral health, or substance use needs. Familiarity with the Orange County Coordinated Entry System (CES), Homeless Management Information System (HMIS), and local housing resources. Experience with staff training, coaching, onboarding, or workflow oversight. Strong documentation, organizational, and case management skills, including assessments, care planning, service coordination, and progress tracking. Ability to manage confidential and sensitive information in compliance with HIPAA and agency standards. Demonstrated proficiency with Microsoft Office Suite, data entry systems, and electronic documentation platforms. Ability to work occasional evenings and Saturdays as needed. Preferred QualificationsThree (3) or more years of experience in housing navigation, homelessness services, CalAIM Community Supports, or related programs. Extensive experience with housing navigation and landlord engagement within Orange County. Knowledge of CalAIM Community Supports, Housing Trio services, Transitional Rent, Medi-Cal systems, and healthcare navigation. Experience assisting clients with public benefits and entitlement programs, including Medi-Cal, CalFresh, CalWORKs, SSI/SSDI, or related services. Experience supervising multidisciplinary teams, managing workflows, and supporting program implementation. Training or certification in Motivational Interviewing, Trauma-Informed Care, Harm Reduction, Crisis Intervention, De-Escalation, Domestic Violence, Suicide Prevention, or related evidence-based practices. Experience conducting community outreach, stakeholder engagement, and partnership development. Experience working collaboratively with healthcare providers, managed care plans, housing authorities, community-based organizations, and government agencies. Knowledge of fair housing practices, tenancy support, and housing stabilization strategies. Intermediate to advanced proficiency in Microsoft Excel and experience with data tracking or reporting systems; experience with Efforts to Outcomes (ETO) or similar case management software preferred.
