Responsibilities • Accurately review, verify, and dispense prescription medications in accordance with state and federal regulations., • Provide medication counseling and education to patients regarding proper medication use, potential side effects, and drug interactions., • Perform medication therapy reviews and assist in optimizing therapeutic outcomes., • Prepare and compound medications as required, utilizing proper aseptic technique and established pharmacy procedures., • Assist with the preparation and verification of IV medications and infusions when applicable., • Conduct patient assessments related to medication therapy and collaborate with healthcare providers regarding treatment recommendations., • Utilize pharmacy software and EMR/EHR systems for prescription processing, documentation, and medication reconciliation., • Support immunization services and medication administration activities in accordance with applicable regulations and organizational policies., • Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times., • Monitor for potential drug interactions, contraindications, and adverse drug events., • Assist with inventory management activities, including medication ordering, storage, and monitoring for expired products., • Maintain accurate pharmacy records and documentation in accordance with regulatory requirements., • Collaborate with pharmacy technicians and other healthcare professionals to ensure efficient workflow and quality patient care., • Participate in quality assurance and medication safety initiatives., • Maintain a clean, organized, and safe pharmacy environment. Qualifications • Doctor of Pharmacy (PharmD) degree from an accredited college or school of pharmacy., • Active and unrestricted pharmacist license in the applicable state., • Strong knowledge of pharmacy practice, pharmacology, medication therapy management, and patient counseling., • Experience in LTC, retail, hospital, specialty, or institutional pharmacy settings preferred., • Knowledge of physiology, anatomy, medical terminology, and pharmacotherapy principles., • Proficiency with pharmacy management systems and EMR/EHR platforms., • Experience with medication compounding and aseptic technique preferred., • Familiarity with immunization administration and medication therapy management services., • Understanding of HIPAA regulations and pharmacy compliance requirements., • Strong attention to detail, organizational skills, and commitment to patient safety., • Excellent communication and interpersonal skills., • Ability to work effectively in a collaborative healthcare environment. Preferred Qualifications • Experience in Long-Term Care (LTC) pharmacy, hospital pharmacy, or specialty pharmacy., • Experience with IV preparation and sterile compounding., • Immunization certification., • Board certification or advanced clinical training is a plus.
