Job DescriptionDescription: Summary of Position: An employee in this position is responsible for overseeing the compliance to OPWDD regulations and Adapt of the Hudson Valley guidelines assigned to the residence. Responsible for the oversight of medical issues at residence, including oversight of direct care staff and LPN (as related to medical concerns). Provide direct support and serves as a role model, advocate and teacher for the people we support. Position Responsibilities: • Promote an environment that encourages and promotes independence and individualization in all aspects of the lives of the people we support., • Responsible for the well-being of the people we support. Exhibits sensitivity to and respects the rights of the people supported, including their emotional and physical needs., • Provide training, supervision, assistance and monitoring in areas related to medical services, appointments, and care to direct support staff., • Responsible for the ongoing Plans of Nursing Service for each person supported to include their diagnoses and ensure staff are trained on the plans., • Monitor the data collection on the eMARs (electronic Medication Administration Records), • Oversee the Licensed Practical Nurse and Direct Support Professionals as related to medical concerns., • Monitor and ensure all staff training/certifications are current at all times, including Medication Administration Cards (MAC)., • Visits each Program at least once per week. Make hospital visits as required., • Ensure the residential setting is always (internal/external) audit ready., • Maintain training as required by agency policies., • Associates Degree in Nursing, • Have the ability to speak, read, and write English fluently which allows full communication with people served; AHV staff; and, 3rd parties., • Have the ability to relate appropriately to people supported with intellectual and/or developmental disabilities., • Computer proficiency, particularly with Microsoft Office applications, and the ability to learn to use other database systems., • Ability to work independently and as part of a team., • Ability to use sound nursing judgment while following AHV policies, practices and procedures., • Ability to operate office equipment (including computers)., • Possess a valid driver license with a driving record acceptable to AHV's insurance carrier and possess the ability to travel to all AHV sites throughout the areas we serve. Job DescriptionDescription: I.Summary of Position An employee in this position coordinates and manages the day-to-day operation of the Day Habilitation program (Open Vistas). This includes the free-standing day habilitation site, individual day habilitation cases and day habilitation without walls programs. The Day Habilitation Coordinator shall oversee the development and growth of the programs with an emphasis on quality and diversity of individualized activities and experiences and shall be responsible for the safety and wellbeing of all the people we serve while in attendance at the program or in the community. There is a direct relationship of the success and growth of the program to the variety and creativeness of the program activities developed. The Day Habilitation Coordinator will embrace the forward thinking ideologies of Adapt of the Hudson Valley?and OPWDD in the movement of individuals from more restrictive and isolative environments to those which foster free thought, individuality and independence. ALL plans developed by the Coordinator of Day Habilitation will embody these tenets. II.?ORGANIZATIONAL RELATIONSHIP The Day Habilitation Program Coordinator oversees two Assistant Coordinators, two shift leaders and 18 Direct Support Professionals (DSP). These staff work on-site and off-site in “without walls” community integrated programs. Requirements: II.REQUIRED DUTIES 1.Shall work with the individuals’ therapeutic team in the development of habilitation plans, behavior plans and safeguards. 2. Shall act as QIDP in the function of approving and certifying habilitation plans., 3. Shall be sensitive to and respect the individual rights, physical and emotional needs of each person and recognize the importance of communicating these needs both verbally and in writing to the appropriate support staff.’, 4. Shall maintain strict fire safety protocols and maintain the fire plan performing regular fire drills in accordance with state regulations., 5. Shall monitor billing on a daily basis and ensure required documentation/notes are done, on-time and with quality., 6. Shall be responsible for fiscal management and timely reporting to the Finance Department. Will monitor and reconcile petty cash funds and funds of people receiving supports., 7. Shall monitor and maintain proper staffing levels., 8. Shall supervise staff and evaluate staff performance, recommend advancement, disciplinary action and/or termination to the Director of Community Services., 9. Shall serve as a liaison (ensuring open lines of communication) between staff, Medicaid Service Coordinators, Residential Staff, Management, Family Advocate, Quality Assurance staff, families and members of the community., 15.
