Mental Health Treatment Aide, Correctional Health Service (GRVC) Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Responsibilities: • Orients patients to the unit policies, procedures, rules, regulations and routine schedule, • Utilizes crisis and/or de-escalation intervention practices, when necessary and appropriate, with patient whose behavior cannot be otherwise supported without such interventions, • Identifies patient characteristics and environmental variables that may trigger agitated behaviors(s) and communicates such information to the treatment team, • Performs patient's checks/rounds as required, • Communicates clinical matters to assigned Mental Health/DOC or other professional staff as required; reports any unusual findings or changes in physical/medical or emotional conditions to their supervisor that may affect the patient or the therapeutic milieu, • Escorts patient to groups or other required locations as directed, • Performs 1:1 monitoring of patients for behavioral management/modification purposes, • Participates in departmental clinical training as required, • May lead or co-lead groups/activities in areas such as anger management techniques and practices, and appropriately documents group activities, • Responds appropriately to patient emergencies and cooperates with DOC staff when necessary to support the safety of the unit, • Participates in regularly-scheduled, in person 1:1 supervision with the MHTA supervisor or designee, • Demonstrates a professional appearance and demeanor at all times, • Performs other appropriate mental health duties as directed by team leaders or supervisors Interface with DOC Staff, • Engage DOC staff when there is any incident of violence on the unit or when contraband is found on the unit or there is suspicion of contraband, • Prior to each tour, appropriately debrief DOC custodial staff regarding problematic patients or issues on the unit Job Hazards: Normal correctional health environment at facilities assigned This position is located on Rikers Island. The Case Planner provides intensive preventive services to avoid foster care placement and ensure the safety and welfare of children in the home. He or she provides the family with services according to ACS standards and requirements. The Case Planner follows a Solution Based Casework (SBC) practice model. SBC is based on three theoretical foundations: family life cycle theory, relapse prevention/CBT theory, and solution-focused family therapy. Partnership with families in each and every case is critical to reducing risks and solutions should utilize prevention skills to achieve those goals. PRIMARY DUTIES & RESPONSIBILITIES: Case Management & Advocacy * Ensure timely completion of SBC training & certification. * Conduct home visits and office visits to assess child safety and risk elements, complying with ACS requirements. Face-to-face contacts should be conducted twice per month, at a minimum and includes home visits (two home visits must be conducted if children 0-2 years old are present) and office visits (e.g., school, doctor’s office, AAFSC), as well as other out-of-home visits (e.g., park). More visits may be required. * Provide case work/case management services to families receiving Preventive Services (12 cases); which includes but is not limited to providing supportive casework, focusing on service goals, and child welfare laws, improving family relationships and bonding, coping skills, parenting, etc. * Ensure joint-home visits are conducted within 5 business days from the date of referral. * Evaluate and assess the current family functioning including child safety and home safety (e.g., assessing other adults in the home, ensuring safe sleep, working fire alarms, window guards, etc.). * Work in partnership with the family to formulate outcomes & service plans with the family. * Ensure all initial intake documents (AAFSC intake, Domestic Violence screening, child trafficking) are completed and processed accurately using AAFSC internal database (i.e., Apricot). * Advocate for the family by linking them to needed services in the community: medical, mental health, entitlements, day care, home health aide services, substance abuse treatment, anger management, etc. and follow up to monitor participation and progress. * In collaboration with the Conference Facilitator, support the facilitation of Family Team Conferences and Service Termination Conferences, as well as other relevant ACS-led conferences. Family Team Conferences must be conducted within 4 weeks of the FASP due date. * Ensure FASPs are launched 30 days prior to the due date and submitted to Supervisor 3 weeks prior to the due date. * Complete Initial, Comprehensive, all Reassessment FASPs, and Plan Amendments for all Advocate Preventive Only (ADVPO) cases offline. * Complete and Submit Child Trafficking Screening for Child Welfare Services and ADVPO cases. * Evaluate walk-ins and community referrals for needed services and potentially for advocacy cases. * Provide case coverage in the absence of a co-worker (as needed). * Schedule and coordinate monthly visits with FSU workers (applicable to FSU cases only). Quality Assurance, Communication, and Documentation * Complete all documents required by ACS in a timely manner (5 business days) including but not limited to progress notes and FASPs using templates in a manner that reflects the progress or lack thereof, including the reaction of the family to services during the review period or contact. * Complete all ACS and internal forms required for case opening and closing as well as obtain consents and forms needed for other case activities. Obtain and maintain collateral contacts with all other service providers including school and medical reports on a regular basis. * Comply with all data entry requirements for database systems including inputting information in a timely manner (PROMIS, Connections, etc.). * Compile and organize appropriate client intake paperwork, surveys, and forms. Maintain casefiles, database entry, and record keeping in compliance with ACS, OCFS, and AAFSC requirements, in alignment with the Solution Based Casework Model guidelines (as needed). * Appropriately follow incident reporting procedures (as needed). * Maintain relationships with ACS personnel, including DCP workers, joint home visit ACS staff, and FSU workers, for ongoing ACS supervision. * Regularly collaborate and communicate with PPRS staff by responding to emails, maintaining an up-to-date Outlook calendar, participating in supervision meetings, and scheduling office days. * Perform other job-related duties as assigned. Trainings, Meetings, and Audit Preparation * Attend training provided by ACS, Solution Based Casework, and other organizations as required (minimum of 42 hours/6 days each fiscal year) and log training by submitting certificates to the Director of Training and Development. * Attend internal meetings as scheduled, collaborating with the multidisciplinary team (e.g., monthly supervision). * Assist in preparation of internal and external audits (COQI, PAMS, safety alerts, etc.) related to the program operation and delivery of services. PRIMARY FUNCTION/PURPOSE: The core responsibility of the Residential Aide (RA) includes the continued assurance of support and care to clients residing in the shelter. Job Description PRIMARY FUNCTION/PURPOSE: The core responsibility of the Residential Aide (RA) includes the continued assurance of support and care to clients residing in the shelter. Position: Residential Aide Reports To: Assistant Program Director Location: 1402 Eighth Avenue, Brooklyn, NY 11215 What The Residential Aide Does: • Maintain professional relationships with clients and client confidentiality., • Practice Universal Precautions/Standard Protocol & Procedures., • Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information., • Update and inform clients of program changes (i.e., food services changes, laundry assignments, etc.) through individual and group meetings., • Assist with client bed-check every evening., • Conduct dorm inspection to ensure client compliance with facility policies., • Intervene and diffuse inappropriate client interaction, as necessary., • Monitor clients’ behavior and report client changes in behavior or personal care to Supervisor and/or appropriate social services staff., • Ensure that clients receive personal care products and laundry products weekly, and replace any personal care items as needed., • Sort, count, and bag bed linens, blankets, and towels (may wash, dry and fold)., • Distribute clean bed linens, blankets, pillows, and towels to clients., • Monitor clients' use of laundry facilities, including running new client’s belongings through the dryer., • Strip former clients' beds and lockers, sort, bag, label and document belongings, carry bags to storage area, and store bags for safekeeping., • Identify, sort, bag, label and document valuable client belongings (i.e., electronics, cash, documents, etc.) and give to Supervisor for storage., • Follow client belongings policy and retrieve personal property of former clients upon their return., • Escort clients, as needed., • Check food deliveries for quality and quantity of items and report any issues to Supervisor., • Prepare kitchen and dining facilities for mealtimes and clean up after each meal., • Heat and serve prepared foods according to Food Standards Policy (i.e., follow menu, set portion sizes, food service hygiene, check temperature of meals, etc.), • Store food according to the Food Storage Policy (i.e., check temperature of refrigerators, rotate food, etc.), • Prepare paper goods, setups and condiments for mealtimes., • Set up coffee and hot water urns before each meal., • May collect garbage in bags and remove to appropriate storage location., • May prepare and serve lunch., • May plan, coordinate and facilitate social and/or peer support events for clients., • Participate in Department of Health Food Protection 15 hour Course, pass exam and become a certified Food Handler., • Must obtain Annual PPD Test., • May be required to become First Aid/CPR certified., • May be required to become certified in overdose prevention. Job Description PRIMARY FUNCTION/PURPOSE: The core responsibility of the Residential Aide (RA) includes the continued assurance of support and care to clients residing in the shelter. Under the general direction of the Shift Supervisor, the RA provides direct assistance, support, and care to clients residing in the facility, as well as supporting, assisting, and enforcing facility rules. This position is responsible for working with clients at our Belnord location in NY, NY. MINIMUM QUALIFICATIONS: • At least a High School Diploma or GED is required, • Must have and maintain a valid NYS security license, • At least two years of experience in Single Adult Shelters and/or Congregate Residential Services is required, • Experience working with families / homeless population is required, • Food Protection certification and CPR/ First Aid certification is required, • Must complete a satisfactory background investigation, • Excellent written and oral communication skills, as well as organizational skills, • Must have incredible initiative, the ability to work well under pressure, and the ability to work both independently and as part of a team to complete complex projects in a fast-paced environment, • Intermediate level of computer skills with proficiency in Word, Excel, and the ability to learn new technologies and databases PRIMARY RESPONSIBILITIES: • Complete new resident Intake arrival packets, exits logs, and daily occupancy forms, • Utilize the Department of Homeless Services (DHS) Client Assistance and Rehousing Enterprise Systems (CARES) to complete Intake Check-ins and Discharge Conditional Intakes, • Review the Nightly Attendance Roster Incident reports, • Receive food deliveries and confirm items received/receipts for accuracy by signing and dating forms, • Report all inconsistencies with food deliveries to the appropriate Shift Supervisor and Program Director, • Distribute meals to residents and dispose of expired food items as outlined by the DHS Food Distribution Guidelines, • Monitor logs to ensure clients are signing the Daily Attendance Roster when entering and exiting the facility, • Provide clients access to rooms and assist in the facilitation and completion of room transfers/swaps, • Collect trash in office and dispose trash in designated areas, • Distribute DHS notices (4002 notices, appointment slips, mail, etc.) to residents, • Adhere to DHS housekeeping mandates by cleaning office space, microwave, refrigerator, and bathroom at the end of every shift; always maintain a clean and neat office space, • Bag and tag client personal belongings when necessary, and report room cleaning and repair needs to Shift Supervisor, • Collaborate with on-site Shift Supervisor to ensure a safe environment for all staff and clients, • Conduct hourly rounds of the facility to ensure all room doors are secured, and clients’ personal belongings are secured, • Conduct safety checks of clients staying in the facility due to illness, mobility issues, etc., • Ensure clients are responsive during safety checks, • Immediately alert medical staff and immediate supervisor of any clients found in distress, • Must be able to rotate shift at the discretion of the DPO to meet the needs of the program, • Conduct operation unit inspections during the new intake process, • Maintain all logs (Attendance, ACS, emergency contacts, sign-in/out logbook) and submit logs on the 1st of every month to the Director of Operations, • On the 1st of every month, create a dead file of all the logs to be submitted for storage, • Adhere to the dress code policy for uniformed personnel and maintain a clean, neat, and professional appearance at all times, • Review all paperwork and logs completed during the previous shift with the Shift Supervisor before the end of tour, • Physically check all unoccupied rooms to determine status of readiness, repairs, and cleanings, • Ensure all clean and ready units have clean linens and towels; alert maintenance team of any items needed, • Complete and submit all paperwork (daily occupancy, exit logs, meal census, NSR, new intake, curfew violations, incidents etc.), • Serve breakfast 8:00am to 10:00am, lunch 12:00pm to 2:00pm, and dinner 6:00pm to 8:00pm, • Finalize Night Signature Roster (NSR) in CARES, • Ensure new logs for the month are in place, • Receive and log incoming meals, usually at 6:00am, • Deliver mail during the 8:00am to 4:00pm shift, • Clock in where appropriate using the time clock system sign-in via the onsite logbook, • Must be available for overtime when necessary, • Required to work additional shifts to ensure coverage when staff is out of the office for various reasons Other Requirements (including Physical Demands): • May be required to work long hours and a varied schedule to meet job demands and deadlines, • Periodically may bend, lift, and carry objects weighing up to seventy-five pounds, • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions EEO Statement – Children’s Rescue Fund is an Equal Opportunity Employer. AmeriCorps members are part of our unconventional programs and services throughout the Center that engage residents and promote public safety. Members work directly with community members to provide services and referrals to address needs centered on economic mobility, housing issues and many other areas. Reporting to the AmeriCorps Coordinator, the Peacemaking Program Aid focuses on providing restorative justice opportunities that emphasize healing, accountability, and community restoration. The Peacemaking Program Aide will work closely with a dedicated team to support clients and community Peacemakers through effective case management and communication. The goal is to create a collaborative justice environment in Red Hook. AVAILABILITY The start date for this position is December 1, 2025, with an end date of September 30, 2026.
