Job Description Job Summary: The Caregiver provides direct personal care and related services in the home that are allowed to be performed under state law. She/he functions under the direction, instruction and supervision of the RN Supervisor. Must drive and have a car. Responsibilities: • Provide direct patient care under the supervision of a registered nurse or licensed practical nurse., • Assist patients with activities of daily living, such as bathing, dressing, and grooming., • Help patients with mobility and transfers, including lifting and positioning., • Monitor and record vital signs, such as temperature, pulse, and blood pressure., • Assist in the maintenance of a clean and safe environment for patients., • Collaborate with healthcare professionals to ensure quality patient care., • Ability to lift and move patients safely and with proper body mechanics., • Experience in assisted living or long-term care settings preferred., • Knowledge of patient care techniques and infection control protocols., • Must drive and have a car., • Valid driver's license for transportation purposes., • Strong interpersonal skills for effective communication with patients, families, and healthcare team members., • Understanding of social work principles for providing holistic care to patients., • Familiarity with dementia care practices and behavior management techniques., • Previous experience in homecare or related fields is a plus.
