Job DescriptionSalary: $25-$31/ Hourly Location: Sacramento County (multiple CORE Community Wellness Center Sites (3x a week)) with some hybrid administrative work Schedule: Full-time, typically Monday- Friday, must be available for urgent operational needs to meet contract requirements and operational needs. Position Summary The CORE Wellness Supervisor leads day-to-day peer wellness operations across assigned Community Wellness Center sites. This role ensures service delivery quality, documentation compliance, staff performance, and contract-aligned outcomes. The Supervisor sets the tone for accountability, professionalism, and recovery-oriented practice. Leadership Expectations Set clear expectations, coach in the moment, and document performance concerns promptly. Maintain reliable coverage and enforce attendance standards to protect contract performance. Use data to manage productivity and quality, not assumptions. Create a psychologically safe team culture while maintaining high standards and direct feedback. Coordinate effectively with TPCP leadership and represent SWS with professionalism. Essential Duties and Responsibilities Provide daily operational oversight of CORE peer service delivery across assigned sites. Supervise Peer Wellness Mentors, including scheduling, coverage planning, coaching, and performance management. Run daily huddles and ensure staff are prepared for program operations, groups, and engagement activities. Monitor documentation quality and timeliness in the EHR and any required systems; correct issues immediately and escalate persistent problems. Monitor service delivery metrics and unit expectations; review dashboards weekly and implement corrective action plans to address gaps. Ensure 90-day outreach and 180-day discharge workflows are implemented consistently and documented in the EHR. Support hiring, onboarding, training, and competency development of peer staff, including tracking peer certification timelines (90 days). Coordinate with TPCP site leadership and support departments through CORE leadership as needed to maintain operations. Respond to incidents and safety issues; ensure incident reports are completed accurately and on time. Provide direct peer services as needed to maintain coverage and support participant needs. Maintain strong working relationships with community partners and represent the program professionally. Other duties as assigned. Required Qualifications Peer Certification Lived experience aligned with peer support values (preferred) and demonstrated commitment to recovery-oriented practice. Minimum 2 years leadership experience and at least 1 year direct supervisory experience (staff and/or volunteers). Minimum 2 years direct service experience in behavioral health, peer support, outreach, or related human services. Strong conflict resolution skills and ability to coach performance in real time. Ability to analyze operational data and translate it into clear expectations and action plans. Proficiency with email, Microsoft Office, and ability to use EHR/data systems reliably. Valid California driver license and proof of auto insurance. Successful completion of DOJ background check. Preferred Qualifications Experience leading peer-certified teams in a Medi-Cal or county-funded program environment. Training in crisis de-escalation, Motivational Interviewing, and group facilitation. Work Environment Work is performed across Community Wellness Centers and community settings. Frequent exposure to crisis situations and high-acuity behavioral health presentations. Requires travel between sites and occasional non-standard hours to ensure coverage. Physical Requirements Ability to lift up to 30 pounds; ability to stand and move throughout site during operations. Ability to respond quickly during emergencies and navigate community environments. Compliance and Performance Expectations Accountability for staff performance, documentation, and service delivery outcomes is core to the role. Enforce policies consistently; exceptions require Program Director approval and documented rationale. Ensure staff follow confidentiality and data security requirements at all times. Complete incident reporting and escalations same day and within required timelines. Model professional communication, boundaries, and adherence to the chain of command. Lead incident response and risk mitigation, including partner notifications and internal documentation. Provide direct operational support at sites as needed to maintain coverage and stabilize performance. Other duties as assigned.

Schedule & Status * Full-Time | Non-Exempt * On-call rotation: 24-hour coverage for a shared 5-day shift with the Program Director and other CSFs * Role includes travel between program sites Leadership & Reporting * Reports to: Program Director * Supervises: House Managers (HM) and Community Living Instructors (CLI) What You’ll Do * Provide direct support and guidance to individuals with developmental disabilities, including those with behavioral challenges, medical needs, psychiatric diagnoses, or forensic involvement, in accordance with individualized service plans. * Support individuals in developing and maintaining independent living skills, including daily routines, community integration, social skills, and self-advocacy. • Supervisory & Leadership Responsibilities (as applicable) Assist in supervising and mentoring direct support staff to ensure high-quality, consistent service delivery. * Support onboarding and training of new staff assigned to individuals on your caseload. * Monitor staff performance and provide feedback related to individual care and service outcomes. * Coordinate and monitor individualized services, ensuring supports align with regional center requirements, service plans, and person-centered goals. * Serve as a liaison between individuals, families, service providers, regional centers, and internal teams to ensure continuity and quality of care. * Assist with behavior support strategies, de-escalation techniques, and implementation of behavior intervention plans in collaboration with clinical or behavioral teams. * Ensure individuals’ health, safety, and well-being by monitoring medical needs, coordinating appointments, and following all care protocols. * Maintain accurate, timely documentation, including progress notes, incident reports, service coordination updates, and compliance records. * Provide guidance, coaching, and oversight to direct support staff as needed, including training on individual support needs, policies, and best practices. * Participate in team meetings, case conferences, and trainings, contributing professional insight and recommendations for service improvement. * Support crisis situations as needed, following agency protocols and collaborating with supervisors and external partners. * Uphold all agency policies, ethical standards, confidentiality requirements, and state/regional regulations.
