Any concerns should be brought to the attention of the partners as soon as practicable. In accordance with the practice timetable, the post-holder will make him/herself available to undertake a variety of duties including: surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, taking action on electronic prescription requests, and dealing with queries, paperwork and correspondence, (including electronic correspondence), dealing with patient test results and medical reports, referral letters, paperwork and other correspondence in a timely fashion Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers Assess the health care needs of patients with undifferentiated and undiagnosed problems Screen patients for disease risk factors and early signs of illness In consultation with patients and in line with current practice disease management protocols, develop care plans for health Provide counselling and health education Admit or discharge patients to and from the caseload and refer to other care providers as appropriate Record clear and contemporaneous consultation notes to agreed standards, primarily by use of the computer system and complete the clinical system clinical templates as appropriate Collect and use data for audit purposes Compile and issue computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) Prescribe in accordance with the practice standards and locally agreed or national guidelines whenever this is clinically appropriate In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
