Arch is an outstanding healthcare provider delivering primary care to homeless and vulnerably housed patients in Brighton & Hove. In addition to a specialist GP surgery, we also provide in-reach services into the Royal Sussex County Hospital and a variety of community contexts which serve people who are without secure accommodation. We are a committed and experienced team with a passion for improving the health and well-being of people who are homeless or vulnerable. We are looking to appoint a prescribing ANP or Paramedic ACP to lead our enhanced outreach service, which delivers healthcare on the streets, in day centres, hostels and emergency/temporary accommodation. This project was set up in October 2022 and requires staff to demonstrate a high level of flexibility, drive and ambition. In this role, you will work closely with a GP, Health Engagement Workers and other members of the Arch team to ensure the success of the project. The project is supported by a specialist GP with regular clinical supervision and will have the support of the wider multidisciplinary network and the Arch team. Movement between settings is often conducted on ebikes to facilitate case-finding. Job description Job responsibilities Clinical responsibilities • To lead the outreach team, reaching and providing clinical care to people who are experiencing homelessness in various contexts around Brighton & Hove and who are struggling to access the surgery; supporting in complex case management of patients with multiple compound needs., • To take clinical leadership of patient care across various contexts, prioritising work effectively., • To effectively engage patients from a variety of backgrounds., • To independently undertake effective comprehensive holistic health assessments for patients including independent assessments of risk., • To take time to listen to patients who may have highly distressing personal circumstances and stories to understand their needs., • To make appropriate referrals for patients as necessary., • To proactively identify the need for mental capacity assessments, safeguarding referrals, challenges to NHS charging decisions within the caseload and take action as required., • To clinically advocate and negotiate on behalf of patients with partners, health colleagues and hospital staff., • To attend case conferences and multi-disciplinary team meetings as necessary., • To develop systems and processes to support the delivery of the project., • To audit and provide reports on the project when requested., • To foster strong partnerships with partner organisations such as CGL, SCFT, Justlife and others., • To manage staff associated with this project, providing developmental leadership., • To represent the outreach team presenting at conferences and other local local and occasional national forums., • To maintain robust systems of data collection, audit and review of all clinical activities, liaising with fellow colleagues in the practice and in the local and national health system as needed to ensure good patient care and service is maintained and to satisfy the standards of external regulatory bodies such as the CQC etc., • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating., • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment., • To maintain your own continuous CPD and ensure a PDP is kept up to date in line with the practice and nursing/clinical team development plans., • Recognise and work within own competence and professional code of conduct as regulated by your relevant professional body., • To maintain your own annual registration with the relevant professional body and adhere to its Code of Professional Conduct. Person Specification Knowledge/Skills Essential • Effective communication and interpersonal skills., • Knowledge and understanding of current national developments and trends in health care and practice development., • Experience of managing clinical risk and experience of identifying and mitigating external risks to outreach working including knowledge around organisational risk registers., • Understanding of clinical coding in general and Read coding in particular., • Implementing strategies for quality assurance and quality audit. Experience of innovative practice or working with new technologies in health care practice., • Experience of TPP SystmOne clinical system., • Experience of Microsoft Office software., • Enthusiasm and passion for inclusion health., • An understanding, acceptance and adherence to the need for strict confidentiality., • Ability to work without direct supervision and determine own workload priorities., • Ability to work as part of an integrated multi-skilled team., • Pleasant, passionate and kind., • Experience of working in accordance with the NMC Code of Professional Conduct (or equivalent)., • Able to demonstrate experience of ongoing complex case management of patients., • Experience of working with people who are homeless with multiple compound needs., • Registered Advanced Nurse Practitioner or Advanced Care Practitioner (prescriber).
