Social Worker – Community Mental Heath Team Swansea Council Full Time | 37 Hours per Week | Hybrid: Office, Community & Hospital Visits Description Swansea Council is seeking a committed and experienced Social Worker to join our Community Mental Health Team (CMHT). This is a fantastic opportunity to contribute to a multidisciplinary team dedicated to providing high-quality mental health support across the community. Based in Swansea, this role involves a hybrid of office-based work, home visits, and hospital assessments. Responsibilities • Deliver statutory mental health services in line with the Social Services and Well-being (Wales) Act 2014 and the Mental Health Act 1983., • Undertake assessments and develop person-centred care plans, working in partnership with service users, families, and carers., • Manage complex cases and act as care coordinator, ensuring effective risk management and safeguarding interventions., • Prepare and present reports, including for Mental Health Tribunals., • Work collaboratively with NHS colleagues and other professionals to support recovery-focused care., • Maintain accurate, up-to-date records and contribute to service development. Requirements • Qualified Social Worker registered with Social Care Wales., • Mental health social work experience essential, ideally within a CMHT or similar statutory setting., • Confident in completing reports and attending Mental Health Tribunals., • Good working knowledge of WCCIS would be advantageous., • Strong communication, organisational, and partnership-working skills., • Must have a valid UK driving licence and be able to travel to clients across Swansea., • Ideally located within or near Swansea, or willing to travel for the duration of the contract. You will be working on a rota basis, which may include early starts, late finishes and day and night shifts and weekend working. You will offer focused support with a range of tasks, including personal care, preparing meals, administering medicine, shopping etc. You will be involved in facilitating activities which are person-centred, which promote their dignity, independence and well-being. You will need to be committed to providing an excellent level of care for vulnerable people with mental health and autism. You will need to be able to work independently this could involve taking sole responsibility for a person we support, ensuring their safety and health are fulfilled. You will need to work within the regulatory requirements as identified within Social Care Wales and Care Inspectorate Wales. You will need to be over 18 to apply for this role. You will be fully supported in achieving this. Experience in care is not essential as full training will be given.
