Position Summary The Health Access & Outreach Coordinator will contribute to the WellSpace Health Infectious Disease Sexual Health Program through case management of patients, collaboration with community partners, navigation process, data collection, marketing, outreach, appointment scheduling, referral processing, and raising public awareness. As part of the infectious disease sexual health team, this individual will help develop culturally appropriate materials focusing on at-risk individuals who might benefit from sexual health education, screening, treatment, and prevention services. The Health Access & Outreach Coordinator will provide sexual health and infectious disease navigation services (screening, treatment, and prevention), including behavior and harm reduction counseling, resource support and linkage to insurance navigation, and appropriate physician referrals to at-risk individuals.
