JOB DESCRIPTION Job Summary The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. Provides needed care in the environment that patients feel most comfortable and are most receptive including home, nursing facilities, and “pop up” clinic. The Nurse Practitioner will be required to work primarily in non-clinical settings and provide medical care to all levels of patients. Some programs may focus on specific populations (e.g., adult and geriatric, pediatric, women’s health). Perform comprehensive medical assessments, order appropriate tests/procedures for diagnostic purposes, formulate treatment plans, obtain specialists' consultations as needed, and do appropriate documentations as required. Primary Purpose The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and the healthcare environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient's condition and the competencies of the employee. Minimum Specifications Education • Must be a graduate from an accredited school of nursing. Experience • Must have two years of nursing experience. Equivalent Education and/or Experience None. Certification/Registration/Licensure • Must have current, valid RN license or temporary RN license from the Texas Board of Nursing; or, valid Compact RN license., • Must have current healthcare provider CPR course completion card from one of the following:, • American Heart Association, • American Red Cross, • Military Training Network., • Cost Center: 63220 Mother/Baby MFM, 63210 Mother/Baby B, 63200 Mother/Baby A, • Must have current Neonatal Resuscitation Program (NRP) certification on hire or placement in the role., • May be required to have additional specialty certifications.
