(i.e. assessments/ Skilled Nursing Note etc.), • Performs physical examination and review of all body systems and documents such accordingly, • Develops an appropriate and effective Plan of Care (POC) to be submitted to the physician for approval and implementation, • Determines medical necessity for other services that could enhance the positive outcome desirable for the case, • Evaluated the patient’s ADL and iADL abilities and therefore need of support services such as home health aide, • Develops and implements the CHHA plan of care when CHHA services are ordered. Revises and signs this care plan the beginning of each certification period, • Supervises the CHHA in accordance with state/federal requirements and documents the supervision without having to be directed to do so, • Orders “other” professional services that are effective and is willing adjust frequency of these services as outcome progresses and the patient’s condition warrants, • Reviews billing processes with patient and/or family advising patient and/or family when co-pay or insurance is not likely to pay for services., • Effectively communicates with patient and family the POC and progression of such. Keeps the patient informed ongoing, • Effectively communicates with other disciplines in the case (case conferencing) to effectively and appropriately problem solve as situations arise, • Communicates effectively with the Director of Nursing/Nursing Supervisor scheduled visits planned and changes to the schedule on a weekly basis, • Communication with the patient’s physician (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities to effect the best means to obtain the desired outcome for the specific medical problem that caused the case to open and/or recertify, • Communicates in the case conferencing sessions to establish best practices for the individual patient’s needs, • Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend., • Submits ALL assessment documentation within 48 hours of the visit, WITHOUT exception, • Submits requests for re-authorization of “more visits needed” prior to third party insurers authorized number of visits expiring, • Coordinates Community Services that may be available to the patient to assist in safe home care needs, • Understands that the fiscal and clinical management of each case is directly linked to the success of his/her office. Assures that visits are not done that cannot be billed, • Participates in the Performance Improvement Committee process as requested to do so, • Performs clinical record reviews (CRR) per HCSF policy in collaboration with the Nursing Supervisor. Submits the CRR timely, • Participates in the monthly staff meeting as part of the requirements of the position. Job DescriptionDescription: Under the direct supervision of the Site Manager and under a matrix system of supervision and guidance of the PCMH Care Coordinator (PCMH) and Director of Nursing, the Certified Medical Assistant (CMA) assists patients and their families in all aspects of their clinical interaction with their provider. The CMA is an integral part of the Care Team and ensures that all patients have optimal health outcomes. Duties and Responsibilities • Greet patient and family cordially at every visit, using cultural sensitivity and customer service skills. At end of visit, escort patient to check out and ensure that concerns and questions were addressed., • Assist providers within the patient care team with the care of patients including obtaining information on reason for visit, medical history, social history, family history, medications, allergies, vital signs, and others. Responsible for entering information into the electronic medical records, verifying accuracy and reviewing answers at subsequent visits., • Ask patients if they have recently seen another provider, received new medications or were recently in the hospital. Ensure that data is entered correctly and calculations for BMI (in adults) and BMI% and Head Circumference according to the growth chart (in children) are correct., • Have a watch with a second hand and a stethoscope when on duty to perform above tasks., • Verify data as above and repeat a blood pressure reading at the end of the interview using a manual cuff if the patient initially had an elevated blood pressure reading., • Follow standing orders for patients with diabetes, patients with hypertension, patients who present with burning on urination, patients who have a delayed menstrual period and others as developed by providers and approved by the Chief Medical Officer and Director of Nursing., • Obtain finger stick samples for glucose, hemoglobin, and hemoglobin A1C as per provider order or following standing orders. Obtain urine specimen for dip or pregnancy test per provider order or standing orders, and urine culture with clean catch technique per provider order. Obtain finger stick sample for rapid HIV test if trained to perform and as ordered by provider., • Inform the provider of results of all tests done in the examination room, chart controls and enter results in the EHR., • Successfully complete competencies for all in house tests done as per CLIS and NJ State Regulations., • Work with providers to determine whether visit is billable to state and if patient is eligible for referral to our contracted subspecialists., • Ensure that every room is clean and specimens are properly disposed of after every patient., • Ensure that, at the end of the day, examination rooms and other clinical areas are clean. Ensure that tables and chairs are wiped down and any equipment, such as the SPOT machines and any wall units are cleaned with antibacterial wipes., • Ensure that counters and exam tables are wiped down and free of dirt and other substances., • Ensure that in every exam room, drawers are stocked and locked when appropriate., • Enter medications into the EHR, listing prescription medications first, followed by over-the-counter medications and supplements., • Assist the provider in recording medications given from the center’s stock by entering date of dispensing, patient’s name, medication name, lot number and expiration date, followed by the provider’s initials. Ensure that medications that have a cost are paid for by the patient before these are given out., • Assist provider in administering ordered vaccines by preparing vaccine in appropriate syringe and in the appropriate amount and made ready for verification by provider prior to administration. Administer as per Certification, as per training and as per provider., • Assist in obtaining labs from fax administrator and attach hard copies to patient documents if needed., • Assist with completion of forms, insurance referrals (to be sent to Referral Specialist if patent has insurance and requires pre-authorization), notes for work or school and other paperwork., • Provide PCMH with a list of supplies as soon as it is noted that the last box of the item has been opened and in use., • Check supplies in all exam rooms daily, check stock in storage areas weekly, and complete list of needed supplies on a daily basis as needed., • Perform other related duties as assigned by the PCMH or Site Administrator and the Director of Nursing., • Participate in annual competencies for the following, but not limited to: translation, vital signs and measurements, doing TB skin tests, all waived POC lab tests, infection prevention and control, sterilization process and instrument preparation, OSHA, immunization administration and reconciliation, standing orders, medical waste management, patient preparation for the visit, cultural competency and customer service. Job DescriptionJob Summary Under the supervision of the Senior Director of Integrated Services and/or Clinical Administrator, the Manager of Peer Recovery Support Services provides guidance, supervision, and operational oversight to Eva’s Village Recovery Community Center (RCC) and Opioid Overdose Recovery Program (OORP). The Manager of Peer Recovery Support Services also functions as the Patient Navigator / Trainer within the OORP program. Job Duties 1. Embodies Eva’s Village’s core values, voice, and tone to provide an exceptional and effective experience for our clients, funders, employees, and volunteers., 2. Oversee, supervise, and monitor the daily operations of the RCC and OORP program, services, and staff., 3. Collaborates with the Senior and Associate Peer Services Coordinators to develop and coordinate activities and services within the Recovery Community Center, ensuring alignment with related programs, departments, and staff to maximize efficiency and maintain compliance with relevant policies, procedures, and standards., 4. Shall attend any training and/or meetings in person or via conference call that are scheduled by DMHAS to support the delivery, sustainability, and effectiveness of OORP services., 5. Maintains effective intra-office and inter-office communication within departments., 6. Shall ensure cooperation and collaboration with the DMHAS statewide evaluator for the OORP, which may include focus group participation; key informant interviews; and individual data reporting (i.e., select social indicators; substance use, overdose and mental health history; patient outreach; services provided, and recovery and treatment follow-up)., 7. In collaboration with the Sr. Director of Integrated Services shall engage in communication regularly with the DMHAS Project Director as well as the State Opioid Treatment Authority pertaining to any identified issues and/or barriers in linking individuals to treatment., 8. Shall engage in the data collection, which is to be reviewed with the Sr. Director of Integrated Services before its submission to the DMHAS Project Director monthly (by the 1st of each month) in a format to be provided by DMHAS. Data shall consist of a minimum of the following: · Number of reported opioid overdose reversals by Emergency Department. · Basic demographics on the individual reversed from an opioid overdose, including a minimum of the following: Age, Race, Hispanic origin, Gender · Number of visits per month by the Recovery Specialists, Number of successful referrals to treatment by Level of Care (LOC) · Number and types of referrals to recovery supports other than substance abuse treatment (i.e. mental health services, housing, employment, etc.) · Number of follow-up phone calls by Recovery Specialists per month 9. Shall submit a Narrative Contract Progress Report to the Senior Director of Integrated Services and/or Clinical Administrator for review before submission to the DMHAS Project Director within 5 working days of the end of each quarter., 10. Maintain client information and ensure up-to-date documentation in the OORP Teams Channel., 11. Providing an evidence-based comprehensive case management assessment that includes life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family strengths/needs., 12. Communicating with the DMHAS Interim Managing Entity (IME) for substance use disorder assessment authorization and subsequent treatment, as needed., 13. Assist participants in connecting with vital community resources, including but not limited to self-help groups, vocational training, educational opportunities, and social support systems that align with their interests and recovery., 14. Empower participants to advocate for their own needs, ensuring their voices are heard and their goals drive the direction of their recovery services., 15. Providing a warm hand-off to appropriate treatment and recovery services, to include referrals to DHS licensed programs that offer Medication Assisted Treatment (MAT). Developing an integrated Recovery Management Plan (RPM) that is consumer-centered and includes strategies for recovery. The plan shall identify priorities, desired outcomes and the strategies and resources to be used in obtaining outcomes based on the case management assessment, 16. Developing linkages and linking individuals to resources that address specialized needs, such as services related to HIV/AIDS, mental health disorders, chronic and acute health problems, pregnant and parenting women, veterans and problems stemming from involvement with the criminal justice system., 17. Advocating on behalf of the patient., 18. Training Hospital Emergency Department staff (when applicable) on the Opioid Overdose Recovery Program., 19. Shall assist the Recovery Specialists when necessary., 20. As the Patient Navigator shall also engage in, but not limited to, the following: a. Will be on-call at all times for the OORP program after the scheduled employment shift. b. Document the integrated case plan in the individual’s case file, as well as document how the warm hand-off was conducted. 21. Performs other relevant duties as assigned by the Director of Integrated Services and/or Clinical Administrator. Qualifications 1. Must possess a master’s degree in health, psychology, counseling, social work, education or other behavioral health profession., 2. Must possess a Certified Peer Recovery Specialist Certification., 3. Experience working within evidence-based practices., 4. Ability to speak, read, and write English., 5. Must be able to work independently and complete assignments with minimal supervision., 6. Good interpersonal skills are required., 7. Must maintain a professional demeanor and perform duties effectively in stressful/emergencies. EXPERIENCE: 1. Must have a minimum of three (3) years of demonstrated experience working with high-need individuals with substance use and mental health disorders. Must possess the knowledge, skills, and experience necessary to competently perform case management activities., 2. Possess the ability to work with individuals affected by substance use disorder and mental health issues to support and strengthen their capacity to achieve and maintain recovery and engage in health practices., 3. Knowledge of all aspects of addiction treatment and how to access it, including the Stages of Change Theory, Motivational Interviewing, and behavioral problem-solving techniques., 4. Knowledge of the principles of recovery for either substance use or mental health disorders., 5. Knowledge of case management activities and applications particular to recovery management; indigenous and community competencies; community resources for social support and how to access each; resources for food, clothing, shelter and other distal needs and how to access; resources for medical care and how to access; resources for mental health care and how to access; mutual aid recovery groups, their functions, values a. and beliefs and how to access; faith-based organizations and how to access. 6. Knowledge of confidentiality rules and regulations related to substance use and mental health disorders, prevention, treatment and recovery supports., 7. Must be nonjudgmental of or adults who have a substance use or mental health disorder., 8. Must have ability to establish empathy with the individual., 9. Must have ability to work with diverse populations and cultural backgrounds., 10. Must be comfortable in working independently in community settings., 11.
