POSITION OVERVIEW Under the direction of the Senior Administrator of Population Health, the Health IT Manager is responsible for aligning the technology development strategies with emerging needs of La Casa de Salud’s primary care operations and value-based infrastructure. In collaboration with operational, clinical and IT leadership, this position will oversee projects related to patient access, integration, interoperability between Nextgen and external platforms, care team efficiency and other applications and projects that will enhance the clinical outcomes of the Primary Care. This role will also serve as the lead Electronic Health Record (EHR) Nextgen liaison to ensure Primary Care is maximizing the system to full utility on both the clinical and practice management components. This position will ensure the delivery of a patient-centric primary care experience, foster a safe, trauma-informed care environment where staff are engaged, develop and invested in the vision and mission of the organization. This is a supervisory position. KEY ESSENTIAL FUNCTIONS Incubator • Work with La Casa de Salud (LCDS) leaders to translate high-level organizational strategies into actionable innovation projects, drive delivery of strategy, designs, prototypes and pilots., • In collaboration with key stakeholders across the organization, leverage design thinking to assist assessing needs, analyzing opportunities, and generate proof of concepts that can be tested to demonstrate value. Then champion the highest value initiatives, developing and presenting the business case around them, and ensure that they are placed with the right team to implement them and see the value realized., • Maintain a current understanding of the evolving healthcare landscape and technology trends patient engagement, mobile health, population management, operational efficiencies, consumer and staff satisfaction, clinical decision support, care pathway automation., • Develop a roadmap and capability for transferring successful innovation projects to operational teams for scaling., • Regularly communicate with senior leadership regarding innovation team capabilities, objectives, value delivered and ROI on projects., • Design and implement processes and systems for operationalizing focusing on quality support and a positive customer/patient/client experience., • Optimize, integrate, and enhance key processes and workflows to assure results comply with business goals and compliance requirements., • Oversee all aspects and phases of concurrent implementations and consolidations with mindset of reducing complexity and creating universal standards., • Provide direct support and training to providers and clinical/non-clinical staff as the application expert for Nextgen Practice Management and EHR systems, • Serve as the Nextgen liaison - identify and troubleshoot application issues, and work collaboratively with end users and other departments to find resolutions, • Work directly with IT production team to ensure technical opportunities and issues are addressed and/or escalated., • Provide recommendations to IT, Informatics, QI and Leadership regarding new commercial opportunities for LCDS in digital health. This may include methods to augment existing offerings or completely new approaches., • Manage relationships with external vendors and partners to maximize utility of product., • Recruit, supervise, manage, develop and evaluate performance of all the Health IT team., • Prepare and present program information to all levels of management within the customer and internal organizations structure., • Attend Board Meeting as needed and present on projects., • Represent the LCDS at conferences, seminars and meetings, as requested., • Masters degree in health administration, public health in healthcare management, business administration, health information management, health informatics, public administration, engineering or equivalent strongly preferred. BA required., • Three (3) years progressive responsibility in management and supervisory experience in the areas of Health IT, operations, implementation, change management, project/program management, • One (1) year experience in Community Health Centers (preferred), • PMP or CCMP license (preferred), • Knowledge of Ambulatory, Outpatient, and in-patient operational workflows, • Design Thinking / human-centered design skills, • Budget management responsibility, • Experience leading innovation-related methodologies - Lean Startup, Design Thinking, Customer Development, • Agile delivery experience (preferred), • Able to deliver high impact presentations to large groups, • Proven experience facilitating large groups/workshops with diverse participants that deliver specific outcomes, • Able to work effectively and coordinate teams that are cross-functional, cross-business unit, and cross-geography, • Strong understanding of the information technology landscape, • Strong people management and leadership skills, • Strong interpersonal communication skills - able to maintain effective relationships at a variety of levels in the organization, • Strong outcome and target-based delivery skills, • A compassionate and trauma-informed care approach to patients is essential Acacia Network provides a comprehensive and competitive benefits package to our employees. Job Description Position Description The Support Services Specialist is responsible for providing guidance and assistance for clients and volunteers of Harlem United’s Drop-In Program shower services in the Integrated Harm Reduction department. They will oversee the flow of people through the shower area, engage with shower guests, and report on the daily operations to the Program Director, Support Services. The Support Services Specialist works closely with staff of the department to ensure clients are engaged through the lens of harm reduction and with a trauma-informed approach. Currently, this position operates out of our Drop-In Center in Harlem, as well as providing shower services at a community partner location in Ridgewood, Queens one day per week. Locations in the future may adjust with program changes. Essential Job Functions The following duties are mandatory requirements of the job: Operations • Set up shower area and welcome area at the beginning of service and break down at the end of the day., • Ensure cleanliness of shower area and other areas used by the program prior to program start, throughout the day, and at the close of service delivery., • Ensure shower stalls are prepared and sanitized before each shower guest., • Work with drop-in staff to ensure clients are signed in according to distribute toiletries and clothing items, etc., • Explain procedures and expectations of shower program to clients, ensuring that they are following established guidelines and using shower area safely., • Engage with clients receiving services, assisting them with referrals to additional services as needed., • Work with other staff in following safety and de-escalation protocols in working with clients., • Track shower services and client engagement using data tracking process created by Program Director, ensure documentation is up to date and accurate., • Enter service delivery details into online client tracking systems and/or funder databases as required by program., • Orient new volunteers, explain procedures, model appropriate interaction with guests. The following duties are to be performed as assigned by the supervisor: • Attend data driven and clinical supervision, as well as departmental, divisional and staff meetings., • Participate in internal and external trainings as assigned by manager and clinical director, as well as required by Human Resources., • This position requires a high school diploma and two years’ experience in services for individuals experiencing unstable housing or with people who use drugs. Candidate should be familiar with housing services, substance use services, and social services agencies within New York City. POSITION OVERVIEW Under supervision of the Program Director, the Case Manager is responsible for providing case management, housing support to participants and monthly reporting requirements that will ensure compliance with the parameters of the program. KEY ESSENTIAL FUNCTIONS • Provide targeted case management services to children, youth and adult consumers living with mental health illnesses and substance abuse disorders and their families/support systems through ensuring access to care, engagement in care coordination of care to obtain the full range of needed services., • Gather enrollment consents, PSYCKES/RHIO consents, and complete screening, baseline-risk assessments, reassessments, plan of care, plan of care updates and notes in accordance with departmental policies., • Demonstrate the ability to clearly articulate, verbally and in writing, the aims and goals of the department to potential patients, community members and staff., • Participate in quality improvement activities, projects and reviews in collaboration clinical team members., • Complete daily, weekly, monthly, or other periodic requests for narrative or quantitative data reports for program review., • Prioritize the homeless population through identification of new sources of potential patients, onsite meetings with patients at their shelter and conduct outreach and engagement presentations., • Meet regularly with supervisor and attend staff meetings and case conferences. Be prepared to discuss case management and operational issues impacting performance and program operations., • Complete and submit daily activity log in accordance with departmental policies., • Ensure patient is attending scheduled medical and social service visits through building relationships with patients and providers. Routine calls should be made to internal and external providers before and after visits to follow up and provide necessary support to the patients., • Maintain four contacts with each client or at a greater frequency as indicated by the risk stratification and plan of care., • Access and respond per agency guidelines to client complaints of grievances, • Conduct outreach and engagement in accordance with policies via phone, electronic methods, and letter and or field work to client/collateral/provider to engage clients or strengthen connectivity., • Help maintain health and wellness and prevent secondary disease complications through provision of health information, support plan of care, and coaching., • Promote and expand linkage development in all areas related to patient care including social determinants (e.g. housing, employment) and monitor the effectiveness of linkages with other service providers via phone, face to face meetings of formal case conferences., • Communicate effectively with Supervisor in identifying strengths, weakness and opportunities of program operations., • Ensure community-follow up to engage the client in care; promote compliance with medical appointments and encourages client self-sufficiency and empowerment., • High School Diploma or GED required., • Associate Degree and Bachelor’s degree preferred., • Minimum of six (6) years of experience in case management., • Must obtain Mandated reporter (2 hours) training/certificate required., • Must complete Integrated Mental Health/Addictions Treatment Training (IMHATT) Certificate within three (3) months of hire date., • Must complete Integrated Mental Health/Addictions Treatment Training (IMHATT) Certificate, • Must be trained in Trauma Informed Care and in Military and Veteran’s culture, • Must be team oriented and possess a positive attitude with a willingness to be flexible and helpful., • Must be able to multi-task with strong organizational skills., • Excellent interpersonal skills and able to communicate both verbally and in written form., • Commitment to cultural diversity and sensitivity.
