PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings. New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens. This is a grant-funded position ending March 31, 2027. Position Summary: We seek a dynamic and strategic Program Manager to lead and strengthen the operations, communications, and network performance functions of the WholeYouNYC Social Care Network. This staff member will oversee two Program Coordinators and play a critical role in advancing program priorities, partner engagement, and network accountability. The Program Manager will be responsible for guiding partner onboarding and communications, directing network performance monitoring, and organizing administrative functions for the network. The role calls for a collaborative, detail-oriented leader with demonstrated experience managing teams, coordinating partnerships, and implementing strategic initiatives across complex health and social service networks. Specifically, the Program Manager, Network Performance will: • Provide direction in the development, implementation, and refinement of program-specific policies, procedures, and strategic priorities., • Oversee and provide guidance for partner onboarding activities, ensuring new organizations are effectively integrated into the network through clear orientation and ongoing support., • Oversee the collection, analysis, and application of network data to inform evaluation, performance monitoring, and decision-making., • Direct the design and implementation of network monitoring frameworks, processes, and tools., • Guide the development of internal and external-facing network communications (e.g., newsletters, partner updates, public-facing materials)., • Manage and oversee the planning, scheduling, and execution of network meetings, events, and partner convenings., • Oversee the creation of high-quality, audience-specific network presentations and program materials., • Direct the development and implementation of network quality assurance and continuous improvement initiatives., • Approve and oversee key programmatic workflows, including communications, purchase requisitions, and compliance-related documentation., • Serve as a liaison with internal PHS teams, facilitating cross-organizational collaboration and information exchange., • Ensure timely and accurate delivery of network work plans, reports, data collection, and performance improvement deliverables., • Lead special projects and strategic initiatives that advance network operations and service delivery., • Supervise and mentor program staff, fostering professional growth and ensuring alignment with organizational goals., • Bachelor's degree from four-year college or equivalent., • Prior work experience in project management, reflecting a strong understanding of project management methodologies., • A strong familiarity with and understanding of the public health, social services and healthcare landscape in New York City., • Proven capacity to balance multiple priorities, meet deadlines, and adapt to evolving program needs., • Ability to supervise, mentor, and develop staff while fostering an inclusive, high-performing team environment., • Highly self-motivated and organized with the ability to work autonomously., • Proven capacity to balance multiple priorities, meet deadlines, and adapt to evolving program needs., • Exceptional communication and presentation skills, with the ability to tailor messages for diverse audiences including partners, funders, and community stakeholders., • Proven experience leading cross-functional teams to successful outcomes., • Detail oriented possessing strong analytical and problem-solving abilities., • Proficiency in Microsoft Office Suite tools (e.g., Word, Excel, PowerPoint)., • Experience with data collection, reporting, and analysis., • Excellent team player with the ability to be flexible, work collaboratively and respectfully both internally and externally and engage in consensus-based decision making., • Dedicated to helping improve the lives of disenfranchised and marginalized communities., • Ability to thrive in a work environment that is respectful, goal-oriented, and focused, and yet is dedicated to achieving work-life balance., • Ability to embrace diversity; possess good people and cultural competency skills. Desired Skills: • Master’s degree preferred., • Prior experience in project launch, partnership building and network coordination., • Prior experience with closed-loop referral technology platforms., • Multilingual preferred., • Knowledge and understanding of business intelligence solutions (e.g., Power BI, Tableau).
