BioMatrix Specialty Infusion Pharmacy is seeking a Part-Time Per Diem Registered Dietitian to see patients in their homes in Syracuse, NY. Work location is subject to change based on business needs. On-Call: Participation in the on-call rotation is a requirement of this position. It is anticipated that an incumbent in the position would be on-call approx. once every eight (8) weeks for two (2) weekdays per month and one (1) weekend. The Per Diem Registered Dietician collaborates with Upstate Home Care clinical teams to ensure optimal nutrition support for patients receiving enteral or parenteral therapy. This role supports qualification of TPN patients, secures appropriate clinical documentation, ensures compliance with insurance guidelines, and provides clinical expertise as needed. Additional responsibilities include assisting with nutrition support services, maintaining formularies, contributing to policy and procedure development, participating in staff education, and supporting reimbursement efforts related to nutrition therapy. QUALIFICATION REQUIREMENTS • Bachelor’s degree in nutrition or related field., • Minimum two years of professional nutrition experience., • Minimum one year of experience in a home care setting., • Current, active registration as a Registered Dietitian (RD) by the Commission on Dietetic Registration (CDR)., • Current, valid state licensure or certification to practice dietetics., • Certified Nutrition Support Clinician (CNSC) through the National Board of Nutrition Support Certification, Inc. QUALIFICATIONS PREFERRED • Experience working with parenteral and/or enteral nutrition therapy in home infusion or home care environments., • Prior involvement in interdisciplinary care teams., • Familiarity with payer guidelines and reimbursement requirements for nutrition support services. Speak slowly and clearly, • Provide assistance in dining room, as needed, • Answer emergency calls PROMPTLY and courteously, and complete required paperwork afterward, • Retrieve soiled linen and personal clothing from resident living areas. Sort and pre-soak linens, as required, before processing through washing machines, per resident’s Plan of Care, • Responsible for general upkeep and cleanliness of a resident’s apartment/ room. Empty waste baskets, wipe up spills, wash dishes, put away clothes, check refrigerator for spoiled food, • Report any unsecured throw rugs, extension cords, and any other fire/ fall hazards to maintain the No Fall Zone, • Observe condition of room/apartment and report anything in need of repair to maintenance/ supervisor, • Document clearly and objectively on residents, • Participate in in-service educational programs, as required by the community and state., • Record all care given and refusals, • Maintain required safety/ building checks, • Obtain resident weights, as needed, • Maintain universal precautions, bloodborne pathogens and biohazard precautions, • Promote positive public relations for the Community, • Provide assistance with toileting, personal care, changing incontinence products, • Assist with use of durable medical equipment (DME) to include walkers, wheelchairs, and canes, • Use good body mechanics to provide occasional assistance with transfers, • Assist residents to perform passive range of motion (ROM) exercises, as per Plan of Care, • Assist resident with braces, splints, slings, hearing aides, artificial breasts, dentures, and TEDS stockings, according to Plan of Care, • Must provide annual verification of a negative TB skin test. Job Description Role and Responsibilities The Special Teams Home Health Aide position for HCR Home Care requires the employee to be available during their designated shift as the agency needs to support needs of the on-call team (cover vacant cases), congregate team (facility-based), Private Pay Team or the Certified Home Health Agency (CHHA) team (shorter cases, more travel, more specialized care), with a minimum required availability of 16 hours per month on the weekends.
