Job Description SUMMARY: The Community Health Coordinator plays a critical role advancing Catalyst Miami’s mission of building power with frontline communities throughout Miami-Dade County to collectively advance justice and achieve shared prosperity. This position joins Catalyst Miami’s flagship Prosperity Campaign and provides bilingual, culturally responsive public benefits enrollment and health care access navigation to Miami-Dade County residents. The goal of this role is to ensure residents have access to affordable health care and income supports (SNAP, TANF/TCA). Catalyst Miami’s public benefits and health care access services are a core component of our economic justice work, given the connections between affordable health coverage, employment, educational attainment, income, and opportunities to build wealth. DUTIES AND RESPONSIBILITIES: • Provide one-on-one direct services with the goal of reducing health inequities for Miami-Dade County residents; services include public benefits application assistance, health care access navigation, and health coaching., • Advocate for equitable access to health care and public benefits within broader economic justice frameworks., • Provide excellent customer service and timely follow-up to all current and potential clients/program participants., • Monitor client needs for systemic trends/barriers to health and collaborate with Catalyst Miami colleagues to work on policy projects to address identified systemic needs., • Identify needs and refer clients/program participants to other resources and services, including other Catalyst Miami’s other Prosperity Campaign services and Policy & Engagement program offerings., • Maintain accurate, complete and timely client records in organization’s data tracking and management systems and adhere to relevant client confidentiality and privacy policies., • Facilitate and conduct group education classes/workshops related to health care access, public benefits, and economic justice., • Participate in community events, outreach, and meetings related to Catalyst Miami’s health care access and public benefits services., • Maintains a client-centered, asset-based approach to direct services, group education, and collaborative policy projects., • Attend local, state and national training to maintain knowledge of current service trends, relevant public policy issues, and other topics relating to health care access and public benefits., • Other duties and responsibilities as assigned by management. SUPERVISORY RESPONSIBILITIES: • This position does not include supervisory responsibilities. QUALIFICATIONS: • Language: Professional working proficiency in oral and written English. Professional working proficiency in oral and written Haitian Creole and/or Spanish., • Education: Associate’s Degree, • Years of Experience: suggested 0-3, • Certification or License: must obtain Certification in the Department of Children and Families Florida ACCESS and Affordable Care Act Certified Application Counselor within 60 days of hire, • Training: must obtain HIPAA, ACCESS Civil Rights, Security Awareness, and any other relevant training within 60 days of hire, • Computer skills: Microsoft Office Suite and working knowledge of web-based platforms CORE COMPETENCIES: • I respond to inquiries and move forward tasks without having to be reminded to do so., • I follow instructions and ask questions as needed. FUNCTIONAL COMPETENCIES: • Coordinates and delivers direct health care access services and supports community members through coaching, benefits access, and health system navigation using an asset-based, client-centered approach., • Advocates for equitable access to health care and public benefits within broader economic justice/stability frameworks., • Facilitates collaboration across healthcare, social service, and community networks to ensure coordinated health care and public benefits access support., • Maintains accurate and confidential client information while aligning service delivery with evolving community health care access and economic stability needs, and policy developments., • Monitors the impact of health care access education, coaching, and outreach efforts on individual outcomes and systemic health care access and economic stability issues. PHYSICAL DEMANDS AND WORK ENVIRONMENT: • Ability to lift weights of up to 25 pounds, • Stretch and bend to organize and distribute materials for Catalyst Miami activities, • Able to sit at desk for long periods of time and utilize computer for extended periods, • Must be able to work flexible hours to accommodate project needs, • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
