Overview: The Quality Inspector will report directly to the site Quality Manager, with indirect oversight from the Sr. Quality Manager, and will be charged with the vital task of conducting and documenting inspections that adhere to the approved Inspection and Test Plans (ITPs). Quality Inspectors play a crucial role in guaranteeing that all reports are meticulously documented, with accuracy and legibility that ensure traceability to their field locations, thus minimizing reporting errors. Quality Inspectors are expected to excel in communication and foster positive relationships with the owner, owner’s representative, and the Company’s site supervision, all to seamlessly advance our quality objectives. Job Description The Crime Victim Assistance Program (CVAP) is a joint program of Safe Horizon and the New York City Police Department (NYPD)that has assigned advocates to every police precinct and Police Service Area (PSA), 86, in total, citywide. Some precincts have two advocates assigned (a crime victim advocate and a domestic violence victim advocate) except for several identified precincts with one advocate. The precincts with one advocate are assigned to work with all victims of crime, including victims of domestic violence. Advocates literally sit side by side with police officers and NYPD civilian personnel and rarely have their own office space. As such, they develop skills where they address the lack of privacy with clients, while navigating a shared space with an outside entity- the NYPD. Advocates are required to report to the precinct/PSA each day for their shift. The Safe Horizon crime victim advocate/domestic violence advocate partners with the police personnel to provide outreach to victims who have filed police reports, or victims who walk into a precinct and seek assistance. The advocate will enter client information into the Case Management System in a timely fashion. The advocate will work with all victims of crime who need help from the NYPD with a specific focus for victims who filed a report as victims of violent felony offense as well as victims of domestic violence who have filed police reports on an as-needed basis. Given the volume in many precincts, advocates will assess high risk and focus on their on-going support for victims identified as such. Responsibilities: • Provide client centered, trauma-informed and culturally centered services for all victims of crime, as well as criminal justice advocacy and support in all phases of the criminal justice process., • Act as a point of contact for hate crime cases and gun violence cases within the precinct, collaborating closely with NYPD personnel, such as the Gun Violence Strategies Partnership (GVSP) and NYPD Hate Crimes Task Force, as well as city agencies including the NYC Human Rights Commission., • Provide client -centered, trauma-informed, culturally centered services specifically for victims of hate crimes and gun violence, as well as advocacy in the community and within various systems., • Share their expertise in criminal and civil legal systems with neighborhood members and community-based organizations (CBOs) as well as with the police., • Conduct roll-call trainings with the NYPD about the role of the advocate and speak at the NYPD Academy or other promotion events addressing topics, information and resources to help NYPD better serve the community., • Collaborate closely with police officers and detectives to ensure timely, coordinated and effective support for all victims of crime; establish and maintain strong working relationships with all precinct personnel to deliver quality services to all victims of crime., • Conduct community presentations and outreach; travel required for in-person Safe Horizon meetings and trainings as well as with community-based organizations and city agencies to develop and strengthen partnerships., • Work with police officers and detectives to identify families that are most at risk to ensure coordination of services for individuals and families., • Conduct visits to a victim’s home wearing a bullet-proof vest, accompanied by NYPD, to assist victims as needed., • Help clients with applying for compensation through the NYS Office of Victim Services (OVS) and provide advocacy with OVS as needed to ensure clients receive compensation in a timely manner., • Provide housing assistance for clients in need that includes accessing the Coordinated Assessment and Placement System (CAPS) and navigating various other systems to ensure clients can find and secure safe affordable housing., • Provide advocacy with various housing systems, including NYCHA and HPD to ensure clients can access safe, affordable housing., • Conduct client centered safety assessments and safety planning suited to the individual’s needs for safety, privacy, and sense of agency., • Provide information and referrals for victims for services such as shelter, mental health treatment, medical care, and other entitlements, as well as practical assistance such as transportation and food vouchers., • Connect and collaborate with all internal Safe Horizon programs and departments that can comprehensively support the client and case plan. Job Description The Crime Victim Assistance Program (CVAP) is a joint program of Safe Horizon and the New York City Police Department (NYPD)that has assigned advocates to every police precinct and Police Service Area (PSA), 86, in total, citywide. Some precincts have two advocates assigned (a crime victim advocate and a domestic violence victim advocate) except for several identified precincts with one advocate. The precincts with one advocate are assigned to work with all victims of crime, including victims of domestic violence. Advocates literally sit side by side with police officers and NYPD civilian personnel and rarely have their own office space. As such, they develop skills where they address the lack of privacy with clients, while navigating a shared space with an outside entity- the NYPD. Advocates are required to report to the precinct/PSA each day for their shift. The Safe Horizon crime victim advocate/domestic violence advocate partners with the police personnel to provide outreach to victims who have filed police reports, or victims who walk into a precinct and seek assistance. The advocate will enter client information into the Case Management System in a timely fashion. The advocate will work with all victims of crime who need help from the NYPD with a specific focus for victims who filed a report as victims of violent felony offense as well as victims of domestic violence who have filed police reports on an as-needed basis. Given the volume in many precincts, advocates will assess high risk and focus on their on-going support for victims identified as such. Responsibilities: • Provide client centered, trauma-informed and culturally centered services for all victims of crime, as well as criminal justice advocacy and support in all phases of the criminal justice process., • Act as a point of contact for hate crime cases and gun violence cases within the precinct, collaborating closely with NYPD personnel, such as the Gun Violence Strategies Partnership (GVSP) and NYPD Hate Crimes Task Force, as well as city agencies including the NYC Human Rights Commission., • Provide client -centered, trauma-informed, culturally centered services specifically for victims of hate crimes and gun violence, as well as advocacy in the community and within various systems., • Share their expertise in criminal and civil legal systems with neighborhood members and community-based organizations (CBOs) as well as with the police., • Conduct roll-call trainings with the NYPD about the role of the advocate and speak at the NYPD Academy or other promotion events addressing topics, information and resources to help NYPD better serve the community., • Collaborate closely with police officers and detectives to ensure timely, coordinated and effective support for all victims of crime; establish and maintain strong working relationships with all precinct personnel to deliver quality services to all victims of crime., • Conduct community presentations and outreach; travel required for in-person Safe Horizon meetings and trainings as well as with community-based organizations and city agencies to develop and strengthen partnerships., • Work with police officers and detectives to identify families that are most at risk to ensure coordination of services for individuals and families., • Conduct visits to a victim’s home wearing a bullet-proof vest, accompanied by NYPD, to assist victims as needed., • Help clients with applying for compensation through the NYS Office of Victim Services (OVS) and provide advocacy with OVS as needed to ensure clients receive compensation in a timely manner., • Provide housing assistance for clients in need that includes accessing the Coordinated Assessment and Placement System (CAPS) and navigating various other systems to ensure clients can find and secure safe affordable housing., • Provide advocacy with various housing systems, including NYCHA and HPD to ensure clients can access safe, affordable housing., • Conduct client centered safety assessments and safety planning suited to the individual’s needs for safety, privacy, and sense of agency., • Provide information and referrals for victims for services such as shelter, mental health treatment, medical care, and other entitlements, as well as practical assistance such as transportation and food vouchers., • Connect and collaborate with all internal Safe Horizon programs and departments that can comprehensively support the client and case plan. We help children stay safe, families remain together, and newcomers navigate life in New York City. While our doors are open to all, AAFSC has expertise in serving Arab, Middle Eastern, North African, Muslim, and South Asian communities. JOB OBJECTIVE: To provide high-quality case planning, case work, and case management services to at-risk families referred by the Administration for Children’s Services. These families may be experiencing a wide range of challenges, including personal/marital/relationship problems, financial stressors, acculturation issues, family destabilization, and/or parent/child conflict. The Case Planner provides intensive preventive services to avoid foster care placement and ensure the safety and welfare of children in the home. He or she provides the family with services according to ACS standards and requirements. The Case Planner follows a Solution Based Casework (SBC) practice model. SBC is based on three theoretical foundations: family life cycle theory, relapse prevention/CBT theory, and solution-focused family therapy. Partnership with families in each and every case is critical to reducing risks and solutions should utilize prevention skills to achieve those goals. PRIMARY DUTIES & RESPONSIBILITIES: Case Management & Advocacy Ensure timely completion of SBC training & certification. Conduct home visits and office visits to assess child safety and risk elements, complying with ACS requirements. Face-to-face contacts should be conducted twice per month, at a minimum and includes home visits (two home visits must be conducted if children 0-2 years old are present) and office visits (e.g., school, doctor’s office, AAFSC), as well as other out-of-home visits (e.g., park). More visits may be required. Provide case work/case management services to families receiving Preventive Services (12 cases); which includes but is not limited to providing supportive casework, focusing on service goals, and child welfare laws, improving family relationships and bonding, coping skills, parenting, etc. Ensure joint-home visits are conducted within 5 business days from the date of referral. Evaluate and assess the current family functioning including child safety and home safety (e.g., assessing other adults in the home, ensuring safe sleep, working fire alarms, window guards, etc.). Work in partnership with the family to formulate outcomes & service plans with the family. Ensure all initial intake documents (AAFSC intake, Domestic Violence screening, child trafficking) are completed and processed accurately using AAFSC internal database (i.e., Apricot). Advocate for the family by linking them to needed services in the community: medical, mental health, entitlements, day care, home health aide services, substance abuse treatment, anger management, etc. and follow up to monitor participation and progress. In collaboration with the Conference Facilitator, support the facilitation of Family Team Conferences and Service Termination Conferences, as well as other relevant ACS-led conferences. Family Team Conferences must be conducted within 4 weeks of the FASP due date. Ensure FASPs are launched 30 days prior to the due date and submitted to Supervisor 3 weeks prior to the due date. Complete Initial, Comprehensive, all Reassessment FASPs, and Plan Amendments for all Advocate Preventive Only (ADVPO) cases offline. Complete and Submit Child Trafficking Screening for Child Welfare Services and ADVPO cases. Evaluate walk-ins and community referrals for needed services and potentially for advocacy cases. Provide case coverage in the absence of a co-worker (as needed). Schedule and coordinate monthly visits with FSU workers (applicable to FSU cases only). Quality Assurance, Communication, and Documentation Complete all documents required by ACS in a timely manner (5 business days) including but not limited to progress notes and FASPs using templates in a manner that reflects the progress or lack thereof, including the reaction of the family to services during the review period or contact. Complete all ACS and internal forms required for case opening and closing as well as obtain consents and forms needed for other case activities. Obtain and maintain collateral contacts with all other service providers including school and medical reports on a regular basis. Comply with all data entry requirements for database systems including inputting information in a timely manner (PROMIS, Connections, etc.). Compile and organize appropriate client intake paperwork, surveys, and forms. Maintain casefiles, database entry, and record keeping in compliance with ACS, OCFS, and AAFSC requirements, in alignment with the Solution Based Casework Model guidelines (as needed). Appropriately follow incident reporting procedures (as needed). Maintain relationships with ACS personnel, including DCP workers, joint home visit ACS staff, and FSU workers, for ongoing ACS supervision. Regularly collaborate and communicate with PPRS staff by responding to emails, maintaining an up-to-date Outlook calendar, participating in supervision meetings, and scheduling office days. Job Description The Crime Victim Assistance Program (CVAP) is a joint program of Safe Horizon and the New York City Police Department (NYPD)that has assigned advocates to every police precinct and Police Service Area (PSA), 86, in total, citywide. Some precincts have two advocates assigned (a crime victim advocate and a domestic violence victim advocate) except for several identified precincts with one advocate. The precincts with one advocate are assigned to work with all victims of crime, including victims of domestic violence. Advocates literally sit side by side with police officers and NYPD civilian personnel and rarely have their own office space. As such, they develop skills where they address the lack of privacy with clients, while navigating a shared space with an outside entity- the NYPD. Advocates are required to report to the precinct/PSA each day for their shift. The Safe Horizon crime victim advocate/domestic violence advocate partners with the police personnel to provide outreach to victims who have filed police reports, or victims who walk into a precinct and seek assistance. The advocate will enter client information into the Case Management System in a timely fashion. The advocate will work with all victims of crime who need help from the NYPD with a specific focus for victims who filed a report as victims of violent felony offense as well as victims of domestic violence who have filed police reports on an as-needed basis. Given the volume in many precincts, advocates will assess high risk and focus on their on-going support for victims identified as such. Job Description Astor’s Prevention Services/Family Connections is a time-limited, evidence-based model that utilizes a home-based approach to address both the concrete and clinical needs of the family. Prevention services utilize comprehensive family assessment tools to identify behaviors related to risk and to develop Core Outcomes that address the identified child welfare concerns within a family. The Case Planner provides in-home counseling services to families in need of support in developing skill and competencies that will enable them to keep their children in the home and out of placement. The Case Planner will maintain a caseload of 12-14 families and provide change focused interventions with families to improve parental functioning, address life stressors, and improve overall familial functioning. This is happen over a course of 6 to 12 month length of service and a minimum of 2 face to face contacts with each family per month. Job Description The Crime Victim Assistance Program (CVAP) is a joint program of Safe Horizon and the New York City Police Department (NYPD)that has assigned advocates to every police precinct and Police Service Area (PSA), 86, in total, citywide. Some precincts have two advocates assigned (a crime victim advocate and a domestic violence victim advocate) except for several identified precincts with one advocate. The precincts with one advocate are assigned to work with all victims of crime, including victims of domestic violence. Advocates literally sit side by side with police officers and NYPD civilian personnel and rarely have their own office space. As such, they develop skills where they address the lack of privacy with clients, while navigating a shared space with an outside entity- the NYPD. Advocates are required to report to the precinct/PSA each day for their shift. The Safe Horizon crime victim advocate/domestic violence advocate partners with the police personnel to provide outreach to victims who have filed police reports, or victims who walk into a precinct and seek assistance. The advocate will enter client information into the Case Management System in a timely fashion. The advocate will work with all victims of crime who need help from the NYPD with a specific focus for victims who filed a report as victims of violent felony offense as well as victims of domestic violence who have filed police reports on an as-needed basis. Given the volume in many precincts, advocates will assess high risk and focus on their on-going support for victims identified as such. Responsibilities: • Provide client centered, trauma-informed and culturally centered services for all victims of crime, as well as criminal justice advocacy and support in all phases of the criminal justice process., • Act as a point of contact for hate crime cases and gun violence cases within the precinct, collaborating closely with NYPD personnel, such as the Gun Violence Strategies Partnership (GVSP) and NYPD Hate Crimes Task Force, as well as city agencies including the NYC Human Rights Commission., • Provide client -centered, trauma-informed, culturally centered services specifically for victims of hate crimes and gun violence, as well as advocacy in the community and within various systems., • Share their expertise in criminal and civil legal systems with neighborhood members and community-based organizations (CBOs) as well as with the police., • Conduct roll-call trainings with the NYPD about the role of the advocate and speak at the NYPD Academy or other promotion events addressing topics, information and resources to help NYPD better serve the community., • Collaborate closely with police officers and detectives to ensure timely, coordinated and effective support for all victims of crime; establish and maintain strong working relationships with all precinct personnel to deliver quality services to all victims of crime., • Conduct community presentations and outreach; travel required for in-person Safe Horizon meetings and trainings as well as with community-based organizations and city agencies to develop and strengthen partnerships., • Work with police officers and detectives to identify families that are most at risk to ensure coordination of services for individuals and families., • Conduct visits to a victim’s home wearing a bullet-proof vest, accompanied by NYPD, to assist victims as needed., • Help clients with applying for compensation through the NYS Office of Victim Services (OVS) and provide advocacy with OVS as needed to ensure clients receive compensation in a timely manner., • Provide housing assistance for clients in need that includes accessing the Coordinated Assessment and Placement System (CAPS) and navigating various other systems to ensure clients can find and secure safe affordable housing., • Provide advocacy with various housing systems, including NYCHA and HPD to ensure clients can access safe, affordable housing., • Conduct client centered safety assessments and safety planning suited to the individual’s needs for safety, privacy, and sense of agency., • Provide information and referrals for victims for services such as shelter, mental health treatment, medical care, and other entitlements, as well as practical assistance such as transportation and food vouchers., • Connect and collaborate with all internal Safe Horizon programs and departments that can comprehensively support the client and case plan. Job Description The Crime Victim Assistance Program (CVAP) is a joint program of Safe Horizon and the New York City Police Department (NYPD)that has assigned advocates to every police precinct and Police Service Area (PSA), 86, in total, citywide. Some precincts have two advocates assigned (a crime victim advocate and a domestic violence victim advocate) except for several identified precincts with one advocate. The precincts with one advocate are assigned to work with all victims of crime, including victims of domestic violence. Advocates literally sit side by side with police officers and NYPD civilian personnel and rarely have their own office space. As such, they develop skills where they address the lack of privacy with clients, while navigating a shared space with an outside entity- the NYPD. Advocates are required to report to the precinct/PSA each day for their shift. The Safe Horizon crime victim advocate/domestic violence advocate partners with the police personnel to provide outreach to victims who have filed police reports, or victims who walk into a precinct and seek assistance. The advocate will enter client information into the Case Management System in a timely fashion. The advocate will work with all victims of crime who need help from the NYPD with a specific focus for victims who filed a report as victims of violent felony offense as well as victims of domestic violence who have filed police reports on an as-needed basis. Given the volume in many precincts, advocates will assess high risk and focus on their on-going support for victims identified as such. Responsibilities: • Provide client centered, trauma-informed and culturally centered services for all victims of crime, as well as criminal justice advocacy and support in all phases of the criminal justice process., • Act as a point of contact for hate crime cases and gun violence cases within the precinct, collaborating closely with NYPD personnel, such as the Gun Violence Strategies Partnership (GVSP) and NYPD Hate Crimes Task Force, as well as city agencies including the NYC Human Rights Commission., • Provide client -centered, trauma-informed, culturally centered services specifically for victims of hate crimes and gun violence, as well as advocacy in the community and within various systems., • Share their expertise in criminal and civil legal systems with neighborhood members and community-based organizations (CBOs) as well as with the police., • Conduct roll-call trainings with the NYPD about the role of the advocate and speak at the NYPD Academy or other promotion events addressing topics, information and resources to help NYPD better serve the community., • Collaborate closely with police officers and detectives to ensure timely, coordinated and effective support for all victims of crime; establish and maintain strong working relationships with all precinct personnel to deliver quality services to all victims of crime., • Conduct community presentations and outreach; travel required for in-person Safe Horizon meetings and trainings as well as with community-based organizations and city agencies to develop and strengthen partnerships., • Work with police officers and detectives to identify families that are most at risk to ensure coordination of services for individuals and families., • Conduct visits to a victim’s home wearing a bullet-proof vest, accompanied by NYPD, to assist victims as needed., • Help clients with applying for compensation through the NYS Office of Victim Services (OVS) and provide advocacy with OVS as needed to ensure clients receive compensation in a timely manner., • Provide housing assistance for clients in need that includes accessing the Coordinated Assessment and Placement System (CAPS) and navigating various other systems to ensure clients can find and secure safe affordable housing., • Provide advocacy with various housing systems, including NYCHA and HPD to ensure clients can access safe, affordable housing., • Conduct client centered safety assessments and safety planning suited to the individual’s needs for safety, privacy, and sense of agency., • Provide information and referrals for victims for services such as shelter, mental health treatment, medical care, and other entitlements, as well as practical assistance such as transportation and food vouchers., • Connect and collaborate with all internal Safe Horizon programs and departments that can comprehensively support the client and case plan. Role Summary The Behavioral Health Coordinator is instrumental in creating a safe and welcoming community by providing direct services and case management to BFLA tenants. This program focuses of providing legal and job training programs for domestic violence victims, residential and vocational programs for individuals with developmental disabilities.
