Job Description Job Title: RN/LPN Reports To: CTI Team Leader Program: Critical Time Intervention (CTI) Location: Tarrytown/Mobile Hours per Week: 35 FLSA Status: Exempt Salary Range: $80,000 - $95,000 annually Summary Description: The RN/LPN will work as part of a multidisciplinary team in the Critical Time Intervention (CTI) program serving adults with co-occurring behavioral health and physical health needs who frequent ED/inpatient settings. The RN/LPN is responsible for assessing basic health and medical needs within their scope of practice, coordinating health care with community medical providers (e.g., primary care physicians, specialists, etc.), rehabilitation, education, and support services. Key Responsibilities: • Perform nursing assessments and document client medical history and current health status., • Monitor and record vital signs and other relevant health indicators., • Collaborate with the healthcare team to develop and implement individualized care plans., • Provide direct client care, including wound care, injections, and assistance with daily living activities., • Educate clients and their families on healthcare plans, medications, and self-care techniques., • Maintain accurate and up-to-date medical records and documentation., • Collaborate with other healthcare professionals to ensure coordinated and holistic care., • Respond to medical emergencies and provide immediate first aid and intervention. Key Competencies: • Proficient in nursing procedures and clinical assessments., • Demonstrated empathy and compassion in providing client-centered care., • Meticulous attention to detail in recording and monitoring client health indicators, • Ability to engage clients through the development of a therapeutic relationship, • Comfort and familiarity with field work – providing services in client homes and the community., • Good verbal and written communication skills Key Performance Indicators (KPIs): • Adherence to nursing standards and protocols., • Positive client outcomes and satisfaction., • Meets prescribed timeframes for response and service delivery as outlined by CTI model., • Completion and maintenance of all required trainings under the CTI model., • Timely and thorough documentation of client care., • Effective collaboration with internal and external partners., • Response to emergencies and adherence to safety protocols. Requirements: • Candidate must hold and maintain current and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure in New York., • Graduation from an accredited practical nursing program., • CPR certification and other relevant certifications as required., • Minimum of 2 years of experience in clinical nursing, preferably in mental health or community healthcare., • Familiarity with electronic health record (EHR) systems., • Strong interpersonal and communication skills., • Ability to work independently and collaboratively in a team setting., • Physical ability to perform nursing duties, including standing, bending, and lifting., • Experience working with diverse populations, • Basic computer knowledge: typing, communication tools, scheduling tools and web browsers., • Proficiency in Microsoft Office suite and database management systems., • Fluency in English (Reading, Writing, Speaking)., • Must be and maintain clearance by applicable regulatory clearances as required., • Legally eligible to work in the United States without sponsorship. Special Considerations: • This is a full-time, 35 hour per week position with a regular daytime schedule that will include an evening shift and rotating on-call responsibilities., • This position will require availability outside of the standard workday, and as such must be ready and available at times to direct and support staff either by phone or in person if needed during the evenings, weekends and holidays., • Hours Per Week, Reporting Structure, and Location are subject to change per program needs Mandated Reporter: This is a Mandated Reporter position.
