Responsibilties: A day in the life of a Registered Respiratory Therapist at Hackensack Meridian Health includes: • Assists in the diagnosis, treatment, and management of all patients with pulmonary disorders in both critical and non-critical areas pursuant to a prescription of a licensed physician., • Performs cardiopulmonary evaluation, monitoring, respiratory therapy treatment techniques, respiratory procedures and patients and family education., • Responsible for the acquisition, analysis and interpretation of data obtained from physiological specimens, performing diagnostic tests, studies and research of the cardiopulmonary system and neurophysiological studies related to respiratory care., • Administers and/or monitors medical gases, mechanical ventilation support, artificial airway care, bronchopulmonary hygiene, pharmacologic agents related to respiratory care procedures, and cardiopulmonary rehabilitation related and limited to respiratory care., • Utilizes the application of scientific principles for the identification, prevention, remediation, research, and rehabilitation of acute or chronic cardiopulmonary dysfunction., • Administers therapeutic Respiratory Care procedures and sets up related Respiratory Care equipment as prescribed by the physician., • Records all pertinent information of provided therapeutic procedures and patient response in the electronic medical record system, EPIC., • Sets up, verifies proper function, operates and monitors all invasive and non-invasive respiratory equipment on patients in need of respiratory assistance., • Participates in the management of patients on life support systems by recommending and performing appropriate changes based on blood gas results and weaning studies., • Responds to and participates in all emergency and trauma codes., • Oversees the clinical performance of Certified Respiratory Care Technicians to assure clinical compliance to department policies and procedures., • Changes and maintains ventilators, ventilator circuits in accordance with the Infection Control Policy of the Department Policy and Procedure Manual., • Runs quality controls on blood gas analyzers., • Monitors and changes all manifolds, including but not limited to emergency oxygen, nitrous oxide and nitrogen manifold cylinders., • Retrieves, cleans and assembles all respiratory equipment using disinfecting solutions and cleaning agents in accordance with infection control protocols., • Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.). The 2025-2026 program term for NYPL After School will run from September 29, 2025 to June 18, 2026. Tutors are not scheduled to work during scheduled program breaks, including holiday closures and days when NYC Schools are not in session, as well as the summer months. Prior to the program’s launch on September 29, Tutors are required to attend orientation workshops during the weeks of September 15 and September 22, 2025. Job Description Summary Streetwork Project is the oldest street outreach program for homeless and street-involved youth in New York City. The program has grown from its beginnings in 1984 as a storefront project to encompass both outreach and comprehensive harm reduction services. The core service is long-term, one-on-one counseling, which is offered in a non-judgmental and holistic manner utilizing a harm reduction approach that focuses on building trust, fostering self-esteem, exploring options and choices, and empowering youth to change their high-risk behaviors. As part of our continuum of services we operate an emergency housing program for homeless youth in NYC, Streetwork Overnight. The Overnight is a 24 bed facility for youth ages 16-20. It is open from 7 pm to 10 am 7 days a week. Casual Youth Advocates work overnight hours to support the youth while they are in our crisis shelter. As part of our continuum of services, we also provide overnight outreach in teams of two seven nights a week (7 pm to 3 am), throughout the five boroughs of New York City. Casual Youth Advocates work overnight hours to meet the most disenfranchised youth while they are in crisis. Casual Youth Advocates work part-time and on-call, filling in for full-time youth advocate absences at the Overnight and Outreach program. These individuals provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). Services provided by casual youth advocates are the same as full-time youth advocates and include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), and groups. Job Description Position Description Harlem United is the parent company for Upper Room AIDS Ministry (URAM). URAM provides the healthcare services from the Federally Qualified Health Center located in the heart of Harlem. We provide Primary Care, Specialty Care, Dental Care and Wellness Care to those seeking diagnosis and treatment. All employees of URAM are responsible to perform their job practicing good risk management, quality assurance, and delivering excellent internal and external customer service. Under the supervision of the Medical Director and in accordance with established policies and procedures, the Internal Medicine Physician delivers high quality comprehensive primary care for homeless, underserved, chronically ill populations including HIV, Hepatitis C, SUD, Diabetes, Heart Disease and other comorbidities. The AIDS institute Guidelines and the U.S. Preventive Services Task Force (USPSTF) are utilized as resources and guides for accepted standards of care. Essential Duties and Responsibilities Essential duties and responsibilities include, but are not limited to, the following: • Provides Primary Care, routine and acute care, for both scheduled, same day and walk in appointments, • Maintains a professional demeanor at all times, including demonstration of sound moral and ethical character, compassion, adherence to all agency standards for dress and decorum, • Demonstrates proficiency in use of electronic medical record, • Complies with productivity expectations under the direction of the Chief Medical Officer, • Initiation of treatment and management of chronic illnesses including, but not limited to, HIV infection, asthma, bronchitis, diabetes, lipid disorders, and hypertension, • Generates appropriate referrals to ancillary and specialty services, • Maintains documentation according to standards of the agency and regulatory agencies, • Documents clinical activities and information, • Initiation/ changes in treatment, • Referrals to specialty services, or social based services, • Prescribing including electronic scripts, • Documents and locks progress notes within 48 hours of client encounter, • Ensure timely prescription writing, order, progress notes, and referrals, • Follows prescribing practices according to agency policy and state regulations, • May administer medication according to the same procedures and protocols applied to the nursing staff, • Performs routine diagnostic and therapeutic procedures according to accepted practice standards, • Interpretation of laboratory studies and follow-up on consultation reports, • Provide health education and counseling during visits, • Adheres to regulatory guidelines in regards to compliance, ie, Coding practices., • Attends patient care related meetings, as necessary, including but not limited to morning huddles, case conferences, • Manages coordination of care for individual clients by working with available resources both internal and external to the agency, • Provide clinical supervision to Advanced Practice Clinicians (PA and NP) as assigned by the Medical Director, • Provide phone on-call coverage on a rotating schedule, • Participates in CQI activities, • Participate in Meetings, • Attends conferences and community based activities, including advocacy efforts to eliminate homelessness, HIV and disparities in care as designated by the Medical Director or designee, • Achieve and maintain HIV specialist status, preferred, • Accumulate a minimum of 10 specific HIV CME credits throughout the year and a total of 25 credits total per year Ideal candidate for this position possesses most or all of the following: • Bilingual preferred (English/Spanish or English/French), • Experience in treating hard to reach populations, including the homeless, 'immigrants, HIV infected, substance users, and mentally ill, • Desire and ability to work as part of an interdisciplinary team dedicated to the delivery of high quality care, • Leadership abilities with trouble shooting and problem solving skills, • Proficient in use of electronic medical record systems and other technology required for use in the delivery of care, • Track record of positive interpersonal skills with staff, clients, and others, • Willingness to work off hours on special projects as reasonably dictated by Chief Medical Officer, • The employee must be able to lift and/or move more than 50 pounds. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye/hand coordination. Requires sitting, standing and walking for extensive periods of time. The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
