Responsibilities: Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for patients with chronic and subacute conditions — such as coma, ventilator dependence, or complex illnesses — that require 24-hour medical supervision. With a focus on comfort, recovery and rehabilitation, CSTCC blends the effectiveness of acute treatment with the supportive environment of long-term care, helping patients progress toward improved function and quality of life. • Perform physical therapy examinations, evaluations, and individualized treatment planning to restore mobility and improve physical function., • Participate in interdisciplinary case conferences, discharge planning, and the coordination of care across the rehab team., • Implement a plan of care for patients and evaluate the effectiveness of it in an ongoing, systematic process., • Supervise physical therapist assistants, rehab therapy aides, and student clinical experiences to ensure quality care and professional development., • Guide therapy and intervention through evidence-based research As part of the Valley’s largest healthcare system, this role gives you the opportunity to apply advanced therapy skills in a subacute setting while helping patients achieve meaningful recovery goals.
