Lead Care Manager – Enhanced Care Management (ECM) | San Joaquin County A leading healthcare organization is seeking a Lead Care Manager to support its Enhanced Care Management (ECM) program in California. This role focuses on whole-person care by addressing not only medical needs, but also social determinants of health such as housing, food insecurity, and behavioral health. The Lead Care Manager will play a critical role in guiding members through every step of their care journey, coordinating services, and ensuring access to vital community resources. Key Responsibilities • Conduct frequent in-person visits in members’ homes, shelters, and community settings to build trust, complete assessments, and address needs in real time., • Develop and implement individualized care plans, coordinating across medical, behavioral health, housing, and social service systems., • Arrange and monitor services such as medical appointments, prescription fulfillment, transportation, home health visits, meal delivery, housing assistance, and job training., • Advocate for members by navigating insurance authorizations, appeals, and access to timely services., • Serve as the central point of communication between members, families, healthcare providers, and community organizations., • Maintain accurate documentation of assessments, care plans, progress, and outcomes., • Research and connect members with community resources that support stability, independence, and long-term health., • Ensure compliance with Medi-Cal, CalAIM, and ECM regulations and quality standards., • Provide leadership and support to team members, contributing to continuous improvement of care delivery practices. Qualifications • 3–5 years of experience in case management, social services, or healthcare., • Strong understanding of Medi-Cal, CalAIM, and Enhanced Care Management programs., • Knowledge of healthcare systems and local community resources in local county., • Skilled in care planning, service coordination, motivational interviewing, and client advocacy., • Strong organizational skills and ability to manage multiple priorities., • Technical proficiency with case management systems and documentation.
