You will be responsible for ensuring that all program participants are provided with the services necessary to promote personal and economic independence, to have skills required to obtain, maintain, and retain permanent housing, and are living in well-maintained units. This is accomplished by performing the following functions: • Adhere to and enforce program regulations, policies, and procedures to ensure all clients receive quality services., • Develop with clients an Independent Living Plan (ILP) by identifying needs and goals and evaluate clients’ progress on short and long term goals., • Provide information, referrals, counseling, crisis intervention, direct service, and advocacy services for all clients., • Develop and maintain a resource network for referrals as related to the needs of the individuals being served in supervised programs. Collaborate with DSS and other service providers to assist clients in fulfilling ILP and program requirements., • Assist clients with direct services such as current and projected client budget, current resume as appropriate, assistance with ADL skills, etc., • Facilitate face-to-face meetings with clients., • Inspect living space regularly to assess client ADL needs and program compliance., • Limit and document improper use of agency resources by clients such as unit damages, furniture, or appliances., • Evaluate, document, and inform the direct supervisor of physical plant needs of the housing unit and/or common areas., • Complete discharge plans., • Submit all internal and external reports in a timely and correct manner as required by funding agencies and/or CHI. Position: Code Blue Shelter Coordinator General Description: The Shelter Care Coordinator is responsible for overseeing the guests of the Warming Center in accordance with the program’s policies, procedures and regulations. The staff are responsible for ensuring that shelter guests are able to have their essential needs met and safety is maintained. In this position, staff will work with guests who may experience mental health issues, substance use disorder, former incarcerated persons, and those with various medical and/or trauma related issues. These services may include providing general information, crisis services, housekeeping, documentation and other duties necessary for the functioning of the Shelter. Principal Duties: • Orientating guests and conducting intakes upon arrival to collect information and assess the needs and strengths of those in need of services., • Providing general information about shelter operations, rules, regulations and assigning beds., • Monitoring shelter guest compliance with shelter rules and requirements, intervening to stabilize crisis situations using de-escalation techniques either directly or with the assistance of other staff. Shelter staff should not attempt to act alone in a situation that could be deemed dangerous., • Monitor sleeping areas to ensure that health and safety requirements are being met., • Housekeeping and maintaining the upkeep of the shelter and common areas., • Reading and reviewing daily logs and making entries each shift. It is imperative to maintain a safe and secure environment for both the residents of the site as well as HRH staff and volunteers., • Property Protection staff are expected to perform the following duties: Physically check vacant rooms at least 2x per night, verify that emergency exit doors are secured, windows on the ground floor are closed and/or secured throughout the night, kitchen facility is locked and secured, laundry rooms are closed, smoking rules are being adhered to, noise violations are addressed, non-approved visitors are escorted off grounds, report suspicious vehicles on grounds overnight, report and/or attend to any slip hazards identified, etc…., • Rules and policies are to be enforced in a compassionate and human manner. Ultimately, the safety of all guests, staff and volunteers must be a primary concern., • Provide assistance to other on-site staff when unusual sightings are observed or situations present themselves. All suspicious activity must be checked out., • Call local police when necessary to assist with conflicts that might occur. Property Protection should never attempt to act in a situation that could be deemed dangerous. Property Protection will become the contact person when the police arrive on the scene., • An accurate account of all incidents will be recorded on an HRH Incident Report Form and submitted for appropriate signatures. Any incident that requires that the police, fire department, CPS or other governmental agency be called should be documented. Principal Duties: • Actively supervise residents/visitors/guests behavior at assigned site., • Conduct intake interviews as assigned to collect necessary and appropriate information needed by the Housing/Property Management Department., • Assist in orienting new participants to the site logistics and provide a general site overview., • Share information with other staff members, and provide relevant information to outside service providers to ensure resident’s needs are being met in a coordinated manner., • Provide an appropriate level of guidance and support to assist residents in meeting their basic needs., • Serve as an advocate by making appropriate referrals to assist residents in reaching their goals., • Monitor resident’s compliance with site/program rules and requirements on a daily basis., • Stabilize crisis situations as they arise. When simple/basic intervention is not effective the Residential Coordinator will reach out to other onsite staff to assist with diffusing the situation. Principal Duties: • Providing general information about program, crisis intervention services, and may screen or participate in the screening of applicants for acceptance into program., • Orienting participants to program and conducting intake interviews to collect information and assess the needs and strengths of program participants., • Developing comprehensive goal-oriented service plan in conjunction with program participant and where appropriate their family members, service providers and/or significant others., • Providing an appropriate level of guidance and support to assist participants in meeting basic needs, addressing their service plan goals, overcoming challenges and coping with disappointment., • Facilitating referrals and serves as advocate on behalf of participant to assist them in securing services, entitlements and support to reach their goals., • Sharing information with other program staff members and collaborating with outside service providers to ensure participants needs are addressed in a coordinated manner., • Providing individual, group and family counseling in accordance with goals and participants’ case plan., • Facilitating or arranging for facilitation of workshops, support groups and other activities to help participants develop living skills, increase interpersonal relations and support systems, maintain healthy life styles and enrich their lives., • Monitoring participants’ compliance with program rules and requirements, intervening to stabilize crisis situations and delivering positive and negative consequences in accordance with program procedures., • Monitoring progress toward service plan goals at regular intervals establishing new goals and strategies as needed., • Participating in applicant interviews, case reviews, staff meetings, training sessions, supervisory meetings, committee meetings, agency events, and other activities as directed., • Monitoring confidential participant files, case notes, records and reports as requested., • Assisting participants in preparing for discharge, developing a discharge and/or aftercare plan, and maintaining agreed upon follow-up contact., • Providing general office support, operation support, and assistance with general program management when necessary and as directed., • Participating in planning efforts to evaluate program’s effectiveness, identify needs and trends, and develops strategies to overcome challenges and enhance program quality. Experience: Two years related work experience with target population. Must have competent reading and writing skills along with strong communication, and basic computer skills. CHI is looking for full-time Resident Support Specialists (RSS) for one of our Dutchess County locations. The Resident Support Specialist ensures the safety and security of all clients, visitors, staff and property. This is accomplished by performing the following functions: • Maintains visibility to residents by wearing Safety Monitor vest and ID badge and inspects site through regular tours of the property. Reviews video cameras and monitors area consistently., • Enforces CHI, site and DSS policies, regulations and rules with residents., • Provides limited crisis intervention to residents when necessary, until CHI professional staff Case Managers, Program Managers or Director of Emergency Housing is able to be contacted and/or intervene. Contacts supervisor and/or the proper authorities as needed., • Maintains appropriate documentation accurately throughout shift, i.e. turn-over, census, logs, incident reports, case notes, etc., • Conducts unit inspections in accordance with OTDA/DSS standards by physically entering each unit to insure proper attendance is recorded., • Accepts resident referrals and reviews rules and regulations with new arrivals including but not limited to the rule of 2 bags per person upon intake. Perform bag searches upon intake, and as required by bag search requirement protocol., • Submits all internal and external reports in a timely and correct manner as required by funding agencies and/or CHI (logs, notes, incident reports). Maintains logs, office and site in an orderly fashion., • Support afterschool/evening activities as needed. Provide oversight of outdoor and recreational events., • Informs Building Superintendent of work site’s physical condition (i.e. dangerous conditions, possible repair needs, supply needs, etc.)., • Preps vacant units for new placements., • Maintains proper usage of cleaning supplies and inventory, • Facilitates and documents evacuation procedures,( i.e. fire drills) as directed by Program Manager, • At the direction of CHI staff (Program Manager or Director of Emergency Housing), acts as a liaison between the agency and the community at large., • Other duties may be assigned by management as needed to ensure the successful operation of the program. Position: Environmental Services Technician – Full-Time General Description: The Environmental Services Technician .is responsible for the upkeep and appearance of all sites as assigned. In addition to landscaping and snow removal, will assist in general housekeeping at all sites and room turnovers as assigned. This position will also be responsible for routine service to HRH owned vehicles and equipment. May be required to shadow maintenance staff to perform basic repairs. All supplies needing to be reordered must be reported in writing to the Environmental Services Manager., • Provide “On-Call” and emergency assistance as needed. This will include inclement weather situations for snow, ice, flooding, etc. This work will likely be performed during the early morning hours, evening hours and may include working on the weekends. Experience: • Education/Experience: Must have the ability to read and write in order to communicate assignments and to understand directions on chemicals and equipment.
