POSITION SUMMARY: • Title: Per diem Care Manager (RN, LPN, or Social Worker), • Location: North New Jersey (Clifton Branch) The successful applicant can be either an RN, an LPN, or a SOCIAL WORKER with 2 or more years of experience or relevant experience working with the senior population. The applicant must possess excellent interpersonal skills and strong computer skills, as well as experience in the field of human services, particularly related to the senior population. The applicant should be committed to working with older adults to improve their quality of life. A plus if they have experience with people with mental illness. The person must have great communication and listening skills, be a team player and possess the ability to interact with a variety of individuals. Must have relationships with various resources in their community. Company Perks & Benefits: • Competitive Pay!, • RN : $50 per hour and up, • LPN: $40 per hour, • Licensed Social Worker: $45 and up, • Flexible Schedule, • Direct Deposit, • call or text the number above to schedule an interview, • by sending an email or message through indeed, • by completing an application on indeed Roles and Responsibilities • Complete in-home assessments and create comprehensive care plans to facilitate aging safely in place, • Work with interdisciplinary team members remotely to assist our geriatric clients, • Collaborate with family members and caregivers to assess needs for appropriate interventions and necessary assistance, • Coordinate client’s medical appointments, prescription deliveries, and transportation, • Advocate for client in all environments, • Coordinate community resource referrals appropriate for each client, • Follow up with comprehensive care plans with the goal of improving quality of life, • Timely maintain and input accurate case records into our software database, • Be part of a cohesive care management team and interact with the Hartford Operations Team., • Attend weekly staff meetings and supervision We are an equal opportunity employer and value diversity at our company.
