Position Summary This position will support the leadership and management team to optimize performance on key indicators, including HEDIS (Healthcare Effectiveness Data and Information Set), UDS (Uniform Data Set), public health initiatives, and internal and external quality goals. This position will support the supervision of Outreach Specialists, who are dedicated to engaging the patient population to improve health outcomes and quality of care. Additionally, this role will support the Clinical Quality Manager. Required Qualifications • Bachelor’s degree or higher education preferred., • Minimum of one year of supervisory experience., • Proficiency with PC-based systems, EHR’s and the ability to learn new information systems and software programs., • Ability to interact effectively with Providers, MAs and other health center staff., • Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills., • Ability to create and deliver presentations., • Demonstrate commitment to providing services for the underserved and working with various people from low-income populations with diverse educational, lifestyle, ethnic, and cultural origins; persons with disabilities, people experiencing homelessness, people experiencing substance dependence, and/or possessing physiological impairment., • Exceptional interpersonal skills., • Excellent customer service experience including active listening, problem solving, and the ability to remain calm in emotional or stressful situations., • Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality., • Adherence to infection control procedures, including standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking, and social distancing., • Assist and support the Center/Department/Program to meet standards of High Reliability. Essential Responsibilities • Work with Leadership, Management, and Care Teams on quality-of-care related activities to improve clinical quality performance measures., • Support data-driven quality improvement strategies., • Use continuous quality improvement processes through tracking and trending data, including clinical analysis and reporting for quality monitoring and improvement., • Support the entirety of the HEDIS/UDS/PCHH Electronic Health Record (EHR) review process to identify targeted measures and other key performance indicators to support optimum metric reporting., • Drive system-wide improvements that impact patient care and simplify processes., • Assist with monitoring mandated quality and performance indicators., • Supervise care engagement staff and program, which is dedicated to outreaching and engaging patients in care., • Provide technical and content-specific expertise to other departments relevant to the identification of tracking, analysis, and remediation of quality concerns., • Develop, prepare, and present reports on the care engagement program and issues to both internal and external stakeholders., • Participate in the preparation of materials and reports for Quality Assurance/Quality Improvement and other related committees., • Assist Providers and Leadership in the development and implementation of innovative programs for improving and sustaining operating effectiveness and efficiencies., • Work with Leadership and Management Teams to ensure that WellSpace Health is meeting and exceeding requirements related to HEDIS/UDS, Pay for Performance initiatives, and any contract compliance quality performance projects. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands and arms to reach. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet. The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care.
