Job Description We are seeking for a dedicated and compassionate weekend Licensed Practical Nurse (LPN) to join our healthcare team. (Friday 3pm-11pm Saturday and Sunday 7am-7pm )In this vital role, you will provide high-quality , person centered patient care Assisted Living/memory Care community residents. For Your expertise will help ensure residents receive personalized, efficient, and respectful care while supporting their health and well-being. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse residents populations, including those with complex medical needs such as Alzheimer's disease, dementia. Your proactive approach and clinical skills will contribute to a collaborative environment focused on excellence in patient outcomes. Responsibilities • Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions to inform care plans., • Administer medications accurately following prescribed protocols, and medication management for pain control or other medical needs., • Document all patient interactions meticulously within electronic health record (EHR) systems like Epic, Cerner, or Athenahealth, ensuring compliance with HIPAA regulations., • Collaborate with interdisciplinary teams to develop and implement individualized care plans for residents with neurological disorders like Alzheimer’s or dementia., • Provide patient education on health coaching topics including medication administration, nutrition. Requirements • Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in your jurisdiction., • Proven experience working in diverse healthcare environments such as hospitals (including ICU or trauma centers), nursing homes, hospice care, or outpatient clinics., • Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation and coding (ICD-10/ICD-9)., • Strong knowledge of physiology, anatomy, pharmacology related to medication administration and pain management., • Experience with specialized care areas including Alzheimer’s/dementia care, memory care units, pediatric or neonatal settings (including neonatal ventilator management), or post-acute care environments., • Ability to perform clinical procedures such as phlebotomy, venipuncture, catheter care, tube feeding management, and specimen collection/processing., • Excellent communication skills for triage assessments and working effectively with individuals with disabilities or developmental challenges., • Commitment to maintaining HIPAA compliance and infection control standards while providing compassionate patient-centered care.
