Overview Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist (PT). Modifies specific treatment procedures in accordance with PT plan of care and Physical Therapy Assistant (“PTA”) scope of practice. Documents all care provided and maintain records per VNS Health policy and standards. * Assists in monitoring patient progress through observation and follows up with Supervising Therapist in accordance with VNSNY policy. * Complies with scope of practice for PTA, including a co-visit with Physical Therapist on all initial PTA visits and keeps PT informed. PT will re-assess patient every 6 visits or 30 days (whichever comes first). * Manages assigned caseload; provides physical therapy services to patients as directed per PT plan of care. * Participates in VNS Health meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews. * Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. * Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals. Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment. * Provides care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to deliver patient care. * Transports and utilizes required technology devices (tablet), infection control materials and supplies using VNS Health designated/supplied carrying case (backpack) weighing approximately 15 – 20 lbs to and from patient homes/care facilities, VNS Health offices and therapists’ home. * Participates in special projects and performs other duties as assigned. Department/Unit: Emergency Medicine General Work Shift: Per Diem (United States of America) Salary Range: $78,773.63 - $122,099.12 The clinical research coordinator is responsible for the organization and management of multiple industry sponsored and Investigator initiated clinical trials. The position is accountable for assisting in the timely and accurate conduct of research studies, being concerned primarily for the protection and care of the patient as a research patient from initiation to completion of study. Activities include participating in the study start up and termination procedures, working with study participants in various study tasks including patient recruitment, screening for eligibility, scheduling, physical assessments and biospecimen collection and processing, as well as data collection and study completion activities. The research coordinator serves as the liaison between PI and research site and the study sponsor and other vendors working on the trial. B. Pre-Study Initiation 1. Attend site pre-study visit/qualification visit meetings and Site Initiation Meeting, whether on site or remote, 2. Attend Investigator Meeting either remote or at location specified by the study sponsor (requires travel), 3. Complete and submit new protocol feasibility questionnaires with research site information., 4. Review confidentiality forms in conjunction with the ORA and obtain appropriate signatures f. IATA training certificates (if applicable for the study) 8. Complete the delegation of authority/duty logs to indicate the site personnel involved with the study and update as necessary throughout the course of the study with either new study personnel or personnel that is no longer involved with the study. 9. Prepare for study site initiation visit a. Schedule visit and all required personnel (PI, sub-investigators, coordinators, research manager, pharmacy, clinical nursing units, operating room staff, etc.) to be present b. Ensure all study required items are on-site (tablets for questionnaires, lab supplies, devices, study medication/investigational products, EKG machine, etc.) C. IRB 3. Prepare and submit annual continuing review reports and site termination report to the IRB of record, 5. Prepare and submit protocol deviations and/or violations to the study sponsor and IRB as necessary D. Patient Interactions 1. Participate in or conduct the informed consent process and HIPAA authorization, per AMC SOP, with research participants through discussion of the study protocol, procedures, timelines, risks, benefits and answering any questions. Obtain the appropriate signatures and dates/time on the ICF form and documents the process and file a copy in the patient’s medical record. Assures that patients sign any future amended versions of the consent if applicable. 2. Develop study schedule and required procedures, timelines, and deadlines with study patient and family, the PI and research team. 3. Coordinate patient follow-up study visits, treatments, testing, procedures, and schedules 4. Conduct patient study visits and required assessments per each individual protocol, 5. Assess patients for eligibility using the study specific inclusion and exclusion criteria., 7. Obtain required biological specimens from patient, i.e., urine, sputum, etc., 8. Perform blood draw to obtain blood specimens, 9. Administer study specific scales; i.e. stroke scales, mental assessments, etc., 11. Perform POC testing as applicable per SOP, i.e. urine pregnancy, urine drug test, blood sugar, urinalysis, 13. Dispense to patient and train patient on completion of study diary, if applicable, 14. Review completed patient diaries with patient, 16. Review oral study medication compliance with patient at each visit, 17. The Technician I is responsible for the installation, preventative maintenance, servicing, and cleaning of our ice machines. This role is critical in ensuring the optimal functionality and cleanliness of our equipment to meet our customers’ service requirements while operating with proficiency. The technician will assist in troubleshooting issues, conduct routine inspections, and execute scheduled maintenance tasks under the supervision of senior technicians. The ideal candidate will possess a keen attention to detail, a commitment to maintaining equipment in good working order, and a willingness to learn and grow within the field of ice machine repair and maintenance. The Technician I is dedicated to driving service success that not only enhances customer satisfaction but also maximizes customer retention through reliable and responsive service. A successful Technician I will drive the following outcomes: • Increase the effectiveness and efficiency of the customer service experience by cleaning, installing and or repairing machines properly the first time, every time, • Increase the effectiveness and efficiency of the customer service experience by maintaining the proper truck stock and vehicle cleanliness and organization standards, • Leave every machine serviced in a condition that makes it easier for other Technicians to follow, • The Technician I performs duties as assigned including the installation of equipment, bagging/delivery of ice, cleaning, repairing, and maintaining ice machines. All levels of Technician - I, II, and III, are responsible for the installation, cleaning, and preventative maintenance of ice machine equipment as required, • Perform comprehensive servicing, installation, and thorough cleaning of ice machines per Easy Ice standard operating procedures to ensure optimal performance and cleanliness, • Communicate effectively with coworkers and customers to ensure all work orders and assignments are completed on time and per Easy Ice standard operating procedures, • Accurately document all actions performed during service visits by taking required photos, completing forms, reports, logs, and maintaining clear records of maintenance activities, • Ensure proper truck stock at all times, avoiding unnecessary return trips and delays in customer service, • Manage the pickup and delivery of ice as needed for service calls, ensuring timely and efficient service for customers, • Identify and report any additional actions required to enhance customer satisfaction, including recommendations for parts replacement, cleaning, or equipment repairs, • Facilitate the processing of accurate payments from customers and assist in the proper shutdown or removal of equipment when necessary, • Assist in maintaining a clean, organized, and safe work environment in the shop when applicable by adhering to all safety protocols and cleanliness standards Education: High school diploma or general education degree (GED) Vocational/Technical Training preferred Experience: • 1-2 years of experience in a customer service-oriented technical role or equivalent combination of education and experience preferred, • Previous experience with ice machines or HVAC maintenance or similar equipment is a plus, • Basic understanding of mechanical systems and electrical components related to ice machines or similar equipment, • Familiarity with troubleshooting techniques and standard repair practices, • Strong communication skills to effectively interact with customers and team members, • Problem solving abilities and a proactive approach to addressing issues, • Ability to manage time effectively and prioritize tasks to ensure service calls are completed in a timely manner, • Skills in maintaining organized records and documentation of service activities, • Demonstrated track record of delivering highest level of customer satisfaction Additional Required Attributes: • High-energy individual with a positive attitude and strong commitment to customer satisfaction, • Willingness to learn and adapt to new technologies and service protocols in the field of ice machine repair and maintenance, • The employee is often required to stand; walk; sit; use ladders; use hands to manipulate, handle, or feel surfaces, parts and tools; reach with hands and arms and stoop, kneel, crouch, or crawl, • Occasionally lift and/or move up to 75 pounds, • Stands for extended periods of time (6+ hours per day), • While the noise level in the work environment is usually somewhat quiet, the worker may be subject to elevated noise levels, • Drive a vehicle for up to 60 percent of the hours worked, • Ability to work flexible shifts and adapt to changing work schedules The above statements are intended to describe the general nature and level of work performed by people assigned to this classification.
