PURPOSE STATEMENT: Preventive Services help keep children safe from abuse, neglect and maltreatment and help them remain at home and in their communities. We partner with at-risk families by assisting them to develop the skills to manage crisis and maintain safety and stability by offering a range of clinical and concrete services to help them thrive. Our culturally competent staff addresses the individual needs of the child, each family member residing with the child, and their social economic environment. Families are seen primarily in their homes and communities. POSITION OVERVIEW: The Case Planner works with the entire family in evidenced based or non-evidenced based model programs. The Case Planner makes safety-focused assessments of the child and family, and provides counseling services, advocacy and refers for concrete services to help stabilize the environment, improve family well-being, and to reduce the future need for child welfare involvement. The social worker works under the direction of the principal to assist and support the implementation of student support initiatives. They aim to provide a school environment conducive to improving student achievement. Responsibilities include, but are not limited to: * Supervise the consistent and fair administration and tracking of student data and create positive school culture; * Contribute toward a safe environment for learning, including ensuring that student conduct is maintained and discipline is enforced fairly and consistently, and updating and implementing the required annual school safety report; * Coach teachers on classroom and behavior management techniques; * Identify and intervene with high-need students; * Assess and differentiates between students with mental health, emotional and behavioral needs and assist in service interventions; * Provide clinical individual and/or group counseling; * Provide comprehensive case management and advocacy; * Support school administration in conducting risk assessment, crisis intervention and mediation; * Participate in Student Assistance Team to mMonitor and evaluate effectiveness of student intervention and action plans; * Foster positive partnerships between the school, families and child serving organizations; * Identify new opportunities to partner with youth service agencies, such as mental health providers, foster care, social services, juvenile justice, and drug prevention and treatment programs; * Collaborate with administrators, teachers and support staff to: * Implement and monitor individual student interventions and action plans * Develop school-wide behavioral management tools * Support attendance improvement initiatives * Advocate for child safety and wellbeing * Perform statistical, narrative and record-keeping duties, including data entry and maintaining case logs * At the direction of school administration, ensure compliance with mandated reporting, confidentiality, social work code of ethics and other school-wide policies and regulation * Strategic management and development of the school, including creating organizational goals and objectives, overseeing annual review, and—in collaboration with Capital Preparatory Schools—envisioning; * ongoing strategies to maximize the effectiveness of the school model; * Implement and follow policies and procedures for student dress code, discipline, and other policies; related to maintaining a positive school culture of learning and achievement; * Serve as a liaison to parents and facilitate parent education and involvement, including reinforcing the need for parents to support the school’s policies and practices for student discipline, dress code, homework, and events; * Undertake administrative and instructional duties as the principal may assign. Skills and Qualifications: * Masters in Social Work; Certification in Social Work * 2-3 years of experience in a school setting preferred * Experience and demonstrated ability in working with children in a teaching, counseling or other applicable capacity (examples include, but are not limited to involvement with community youth organizations, supervisory roles in national youth organizations, and mentorship programs); * Demonstrated successful teaching experience; * Exhibited leadership in working with professional staff, students, and the community; * Agreement with the academic goals and philosophy of the charter school; * Commitment to accountability and student discipline; * Demonstrated success in encouraging parental involvement. * Demonstrated excellence in verbal and written communication. Compensation: This is a full-year, full-time, exempt position with a competitive salary range of $79,200 to $94,656, commensurate with experience. Job Description Title: Homeless Outreach Worker Reports To: Homeless Outreach Supervisor FLSA: Non-Exempt Status: Full-time Supervisory Responsibility: Not Applicable Who we are? CHOICE is an agency that takes pride in providing case management and advocacy services for our clients with mental health or physical health illnesses. We specialize in providing quality coordination of care, referrals, peer support, groups, and other related services in a friendly environment. Purpose of the Role: Our Homeless Outreach Worker (HOW) assists with services to individuals living with a mental illness and/or experiencing homelessness. Our Homeless Outreach Worker is also expected to engage individuals who are experiencing homelessness either on the streets or in shelters to provide advocacy, assist with connection to benefits, and housing. Essential Functions of the Role: Upon assignment of a new client, proceeding within the required guidelines, the Homeless Outreach Worker is to immediately contact the client, set an appointment to meet with the client at a CHOICE office or off-site to complete all required paperwork. For all assigned cases: · Set client goals and align care activities in accordance with these goals. · Monitor progress of goals, adjust care plans accordingly. · Collect and enter data and notes accurately, thoroughly and timely, documenting activities and outcomes into case management systems. · Plan and execute care activities in accordance with client goals to minimize any potential issues with billing and reimbursement to the agency. · Plan and utilize time so that needed outcomes for the client are achieved and completed within the time frame set by the Supervisor. · Proactively communicate with Homeless Outreach Supervisor on any emerging issues and needed adjustments. · Connect with each assigned client minimally 4x per month. · Review assigned caseload with Homeless Outreach Supervisor 2 times per month to ensure effective case management (as noted above); make adjustments as needed. · Develop and maintain productive and results-oriented relationships with mission-critical persons at outside organizations. · Attend meetings, as necessary, at the Department of Community Mental Health, to review client cases. · Complete street outreach assessment (VI-SPDAT)as requested by HOW Supervisor. · Using the full capacity of one’s ongoing experience and training, demonstrate the progressive ability to problem solve, advocate, mediate and handle increasingly complex tasks related to care coordination. · Do not settle for “because that’s the way it has always been done”, rather be fearless in the pursuit of excellence and achieving the needed outcomes for our clients and the agency at large. · Other activities as assigned.
