Job Description Position Summary: First Judicial District Training Navigator is responsible for training staff of the First Judicial District Court of Pennsylvania in behavioral health topics. These trainings are designed to increase the behavioral health literacy of First Judicial District Court personnel by, helping them to recognize signs that individuals they serve may be experiencing behavioral health challenges and refer them to the BHJD Resource Hub for connection to appropriate supports. Key trainings center on recognizing signs of behavioral health issues (e.g., Mental Health First Aid, Hearing Disturbing Voices), suicide prevention (e.g., Question, Persuade, Refer), and trauma-informed/trauma responsive care. This role is funded by a grant from the Pennsylvania Commission on Crime and Delinquency. First Judicial District Training Navigator has a secondary responsibility to provide navigation support to the Philadelphia Police Department’s Behavioral Health Unit (BHU). The BHU is comprised of two programs, Crisis Intervention Response Team (CIRT) and Police Assisted Diversion (PAD). Navigators help the BHU "navigate" the maze of the court system, treatment programs, and community supports to ensure the individuals the BHU engages with are successfully linked to the appropriate care and services. Navigators also work to reduce the barriers that keep individuals with mental health needs from remaining in the community, getting timely access to treatment, and diversion from incarceration when appropriate. Job Description Position Summary: The Behavioral Health and Justice Division (BHJD) seeks qualified candidates who are passionate about using data and information to effectively and efficiently inform policy, program, and intervention creation and evaluation for Philadelphia’s who find themselves at the intersection of the behavioral health and criminal justice system. To assist in accomplishing these goals, BHJD has developed the position of Data and Information Analyst. In this position, the Analyst will be responsible for researching, collecting/cleaning data, and performing statistical analysis needed to monitor trends and patterns of individuals served by this division. Job Description POSITION DESCRIPTION: PHYSICAL THERAPIST POSITION SUMMARY: The Physical Therapist (PT) is a licensed health care professional that administers Physical Therapy to adult patients requiring part-time, intermittent visits and assesses their therapeutic needs. The therapist works as a member of an interdisciplinary rehabilitation team serving as care managers with responsibilities for assessing, implementing, coordinating and evaluating patient care whenever nursing is not active with the patient. The PT supervises the physical therapy assistant (PTA) who has been approved by the Commonwealth of Pennsylvania to work in an indirect supervision setting and the home health aide if nursing is not active with the patient. RESPONSIBLE TO: Director of Professional Services or designee. I. A. Makes the initial visit within seventy-two (72) hours of referral unless physician order, care needs or patient request dictate otherwise. B. Reports to the case manager a summary of the patient's condition including: 1. Problems concerning the patients and how these may be resolved., 2. Changes in patient needs and how these can be met., 3. The need for involvement of other members of the health team such as nursing, HHA, speech/language pathologist, occupational therapist, and social services. C. Communicates with health team members to achieve interdisciplinary, coordinated care. D. Calls the supervising nurse or designee with any immediate patient clinical needs. E.
