Job Description:\n\nTitle: Day Services Qualified Intellectual Disability Professional / QIDP Reports: Director of Day Services Department: Day Services Location: Day Services, NY Status: Exempt, Management POSITION SUMMARY: The Qualified Intellectual Disability Professional / QIDP is responsible to assist and ensure the overall operations and services are being delivered according to state and federal regulations, agency policy and procedure and according to the agency’s mission. The role of the Qualified Intellectual Disability Professional / QIDP is responsible to assist in the overall coordinating and managing the overall person-centered services for assigned individuals. The Qualified Individual Disability Professional (QIDP) serves to provide leadership, direction, and training in order to supervise the overall development of a specific program. QIDP also supervises the rehabilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, develop implementation strategies which outline services and supports for individuals so they may work towards the outcomes detailed in their Personal Plan. The QIDP supervises the treatment process in the assigned day programs, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. The QIDP is responsible for ensuring service provision by all disciplines represented on the treatment team. The QIDP meets routinely with other Team Clinicians for coordination and integration of services. The QIDP coordinates with the Program Coordinator(s) and staff. The QIDP monitors provision of individual and group program services as well as the maintenance of a therapeutic milieu which is conductive to their health and well-being. The QIDP ensures provision of integrated services to individuals by fostering within each Team an awareness of the need for service integration; Coordinating service delivery and planning activities with day program services; and Monitoring service provision by agency clinical staff, consultants and community-based vendors, ensuring services are in accordance with IDT treatment recommendations. The QIDP ensures provision of a therapeutic environment through regular review of employee work schedules and performance, initiating corrective actions when necessary to ensure provision of coverage levels consistent with the treatment needs of the individuals and regular monitoring of the program setting to ensure maintenance of an atmosphere conducive to individuals' development and well-being, where the rights of each individual are upheld and respected. The QIDP regularly records progress of individuals, and documents all changes in Plans of Care / ISPs according to procedures established by the agency. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES: Employee must possess the ability to interact with individual and staff professionally and ethically, understand and implement service plans, and to complete all required documentation clearly, legibly in a timely manner. Review deficiencies with IDT and follow-up with appropriate parties.Ensure that all Life Plans, Staff Actions, Monthly Notes, Daily Service Documentation data complies according to OPWDD and OMIG requirements.Notify team members, parents, guardians, relatives and day programs of all team meetings, reviews and apprise them of all habilitation changes affecting individuals.Participates in the Staff Development Program, providing training to subordinates to increase competencies and foster a team approach to service delivery and provides specific instruction to Program Coordinators and Assistant Coordinators to enable them to develop programs, including (but not limited to) ADL, ILS, Recreational & Social Programs to meet individuals' needs, and for IRAs habilitation activities based on each individual’s 'valued outcomes.Provides training to Program Management and DSPs in program implementation and documentation and supervises the development and implementation of each individual’s Plan of Care / Life Plan in accordance with regulations. Provide training to staff on Habilitation Plans to ensure the appropriate implementation of plans.For new individuals, works with the Intake Department and completion of a required documentation for compliant intake procedures Manage and arrange admissions and/or discharges and will work closely with intake, program staff, family members, and agency representatives in effectively coordinating these processes. Ensure admissions and discharge procedures are followed according to state regulations and agency policy; along with the team, review documentation for admissions and partake in Intake Meetings. Assist with service development and increase bed-capacity.Regularly monitors implementation of Plans of Care, initiating revision when: Current Goals / Objectives have been achieved; A Goal / Objective / Day Hab is determined to be inappropriate due to a change in the individual’s status, functioning level or interests; changes are required to meet regulations; (or) the individual or another team member requests such review/revision to better meet identified needs.Coordinates and ensures the overall development, periodic evaluation and review of each individual's individual plan of care and supervises the delivery of training and rehabilitative services relative to this plan of care.Reports regularly to the Supervisor on issues of program compliance, noncompliance, and remedial actions taken to correct deficiencies and identifies needs and requests supports, assistance or direction to meet service needs and ensure program compliance.Participates in clinical reviews, incident review and human rights committee meetings and other meetings as directed by Supervisor. QUALIFICATIONS: Required:Must have at least four experience years working with Life Plans, Staff Action Plans/Habilitation Plans, monthly summaries, service documentation and individuals’ goals and valued outcomes in a managerial/supervisory role.Must be experienced in handling confidential information in the strictest manner, can multi-task and analyze problems and determine corrective measures. Coach Across America is based on a simple premise: That highly trained coaches can be among the most transformative adults in young peoples lives and inspire their success on the field, in the classroom, and in life. What youll do As a Coach Across America AmeriCorps member, you will coach teams and/or programs for elementary to high school age youth. You will be trained on how to become a Sports Based Youth Development Coach to positively impact youth mental and physical well-being. You will monitor youth progress and work with program and/or school staff on sport practice planning, recess facilitation, PE class instruction, and/or small group spaces. We are seeking a Mental Health Counselor for the Crotona Park residence which is The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. The majority of Crotona Park residents will be referred by the New York City HIV/AIDS Services Administration (HASA) and some by the New York City Housing Authority (NYCHA). The Case Manager/Mental Health Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population. About the Role In order to achieve its ambition of serving 80,000-100,000 Fellows over the next decade, Braven is hiring an Associate Director, Workforce Strategy, who will support students towards strong post-college career outcomes. As a member of the New Jersey regional team, you will serve as a strategic career coach to students attending Kean University who have completed the Braven Career and Leadership Accelerator Course (the Accelerator). This role has the opportunity to accelerate Braven’s regional and national impact. This role is on the New Jersey regional team and reports directly to the Senior Director, Kean University. Position Overview The IT Manager reports directly to Canyon's VP of Art & Technology and leads all technology operations for the institution. As Canyon's first IT Manager, this individual will establish and manage all core IT infrastructure while working alongside specialized technical teams-including AV engineers in Exhibitions and conservation technicians in Canyon's conservation center-to ensure seamless integration across all technology systems. KEY ESSENTIAL FUNCTIONS • The head teacher of a given group of children:, • To plan and execution of the Preschool component of La Casita III for children which takes into account the educational, social, developmental and health needs to the group, as well as the individual child., • To share responsibility for keeping equipment and material accessible and in good condition in order to encourage maximum constructive use by the children, • To share responsibility for removing or reporting any damaged or hazardous equipment to the Child Education Coordinator or other designated person so that necessary action may be taken., • To share responsibility for planning and supervising an appropriate arrangement of classroom equipment and furniture designed to promote optimal program functioning, • To assist in the overall planning for the best use of shared in relation to the total center program, • To assist the Department Manager in planning the use of community resources to enrich the educational content of the children’s program, • Responsibility for Professional Growth, • To share with classroom associates their mutual knowledge and understanding of La Casita III Program and the sponsoring agency of which the center is a part of, • To promote his or her own professional growth of development through participation in regularly scheduled individual conference, classroom group conferences, staff meetings and Early Childhood Trainings, • To share with other classroom personnel the performance of routine chores related to all areas of the program, • To participate in objective discussions on his/her own professional performance in supervisory conference with the Department Manger, • Developing and Maintaining a Positive Working Relationship with Parents, • Adhering to the approved plan by which information concerning the children and program can be shared with the parents, • Report any suspect Ensure children are monitored for signs of child abuse or maltreatment and reports of incidents must be given to the appropriate authorities, • Attending and participating in parenting education groups and training, • Respecting the confidential nature of all information about parents and children, • Maintaining good working relations and objective attitudes with other center personnel, • Meet all applicable licensing regulations, • Other related duties, as assigned, consistent with the above and including the fulfillment of full program responsibility for the group in the absence of the group teacher, • Masters, BA in Early Childhood Education, or Related field, • Teaching State Certification, • Must have experience working with children or caring for small children age 0 through 5 years old, • Consistent with the above and including the fulfillment of full program responsibility for the program in the absence of the department manger Acacia Network provides a comprehensive and competitive benefits package to our employees.
