(aging population that wish to live in their home, NOT in a nursing home) AND TBI survivors. Salary commensurate with experience. Bachelors AND 3+ years experience required Looking for a Case Management Supervisor to work a part time caseload (see Service Coordination) job duties below. The supervisor is expected to meet any waiver participants prior to the completion of the Revised Service Plan (RSP) developed by a Service Coordinator under their supervision. Any visits that exceed 10 in a month will be assisted by the TLC Directors. The Supervisor will have supervisory meetings with staff on at least a monthly basis, and review and sign-off on all service plans and monthly notes. In addition, they will monitor the quarterly tracking spreadsheet to ensure proper time management by staff. A supervisor MAY maintain an active caseload of waiver participants up to 25. However, this caseload must be reduced from the maximum limit allowed in relation to his/her supervisory responsibilities. The Service Coordinator Supervisor must complete a monthly face to face visit with each TBI waiver participant on their caseload. They must meet with the participant in their home at least quarterly. All Intensive Case Managers have at least 2 years clinical experience, which includes client direct contact experience. function as an advocate, facilitator, outreach coach, educator, care coordinator, and motivational counselor for members and their families for members who have complex behavioral health and or medical conditions. The role of the Intensive Care Manager includes, but is not limited to the following tasks: Position Responsibilities: • Providing a timely outreach to new referrals, • Engaging members into the program by providing compelling rationale on the benefits of the program to fit the unique member's needs., • Completing members needs assessment to determine appropriate services and inform the care plan., • Developing an individualized member centric comprehensive care plan with input from the member, provider, and family. The individual goals include recovery and resiliency, decreasing symptomatology and/or increasing functional ability in areas such as self-care, work/school, and family/interpersonal relations to reduce barriers to treatment., • Providing monitoring and reviewing of cases through planned outreach, incoming contacts, care coordination and utilizing rounds, weekly reports, and individual supervision., • Rounding or staffing with a supervisor takes place once per month at a minimum for difficult or challenging cases., • Providing consultation and coordination with the behavioral health or medical providers, facility or family members, community agencies, or involved medical practitioners regarding treatment and/or treatment planning issues., • Providing motivational counseling and encourage self-advocacy to help sustain members' commitment to their care plans and treatment adherence., • Coordinating and consulting with the Care Manager as necessary. Attending regularly scheduled rounds to consult with a psychiatrist or health plan staff and discuss cases and the need for continued intensive care management and outreach. As well as our Article 31 Mental Health Outpatient Treatment and Rehabilitative Services. The Nurse provides screening and monitoring of key health indicators and health risk, as well as clinical monitoring for adverse effects of medications, including monitoring for metabolic syndrome. The Nurse will coordinate completion of required steps for initiation of Medication Assisted Treatment (MAT). The Nurse will serve as a consultant to other team members and may take over some of the work needed to adequately coordinate care if the management of a particular medical condition becomes complex. Key Responsibilities: • Provide initial health assessments for all new clinic clients and annual follow-up assessments., • Coordinate care for medical issues, including all post-admission medical testing for hepatitis, HIV, tuberculosis, or STDs on site or by referral to external medical providers, arranging for physical examinations of clients who have not had one within one year prior to program admission, scheduling appointments, and liaising with medical providers to obtain records., • Acts as Health Coordinator for the program, providing health education, risk reduction, counseling, and referral service to all clients, specifically HIV/AIDS, tuberculosis, viral hepatitis, sexually transmitted diseases, and other communicable diseases., • Coordinate and monitor usage of prescribed MAT therapies and psychiatric medications such as clozapine (obtaining lab results, reviewing them, and faxing prescriptions), as needed., • Administers antipsychotic medications to clients, as needed., • If needed, administer medication- intramuscular injections (IM) according to Rx order(s) as documented in EHR., • Collaboration with the prescriber on medication refills., • Conduct toxicology testing, as needed., • Obtain and document lab results when clients have labs drawn by outside providers., • Participation in the clinic team meeting, administering injections as prescribed., • Maintaining supplies for the med. room, communication with pharmacies., • Experience understanding various evidence-based models and theories pertaining to substance use and co-occurring disorders, psychoeducation, motivational interviewing, and recovery., • Psychoeducation about medical issues to all clinic clients, individually and in groups., • Follow the established agency policies and maintaining appropriate professional standards., • Complete all required documentation on time and accurately. This includes but is not limited to, progress notes, health screening assessments and care plans., • Ensure client engagement in health and wellness practices, as well as chronic disease management to reduce hospitalizations and improve quality of life., • Responsible for adhering to all standards and guidelines established by applicable regulatory authorities., • Other related duties. Position Summary: The Clinical Supervisor of the Wellness Center is responsible for overseeing the day-to-day operations of The Arc Westchester’s Article 16 Clinic, and its clinical programs and staff. Reporting directly to the Assistant Executive Director of Community Innovations, the Clinical Supervisor provides leadership for and direct oversight of the array of clinical services and supports provided to individuals with disabilities and their families including PT, OT, SLP, psychotherapy, psychiatric services, and evaluations.
