Below are the position details for your review: Position: Nurse Practitioner / Physician Assistant Employment Type: Full-Time Compensation: $159,000 – $187,000 annually Practice Setting: Community / Integrated Primary Care Role Overview Provide comprehensive primary care services within an integrated health center model, emphasizing prevention, chronic condition management, behavioral health collaboration, and community-based care delivery. Key Responsibilities • Perform comprehensive assessments, diagnoses, and treatment planning, • Prescribe and manage medications within scope of practice, • Deliver preventive care, primary care, and ongoing health maintenance, • Conduct physical exams, review vitals, and evaluate medical histories, • Educate patients on disease processes, medications, prevention, and adherence, • Collaborate with interdisciplinary teams including behavioral health and wellness staff, • Refer patients to subspecialists as appropriate, • Participate in after-hours on-call rotation, • Maintain accurate and compliant clinical documentation, • Engage in team meetings, peer reviews, and quality improvement initiatives, • Assist with coverage for other clinicians as needed Qualifications Required: • Master's degree or higher as a Nurse Practitioner or Physician Assistant, • Active California NP or PA license, • Current DEA registration, • Minimum 2 years of clinical experience, • Clinical background in Family Medicine, Internal Medicine, or Pediatrics, • Bilingual proficiency required in at least one of the following: Mandarin, Cantonese, Spanish, Tagalog, Farsi, or Vietnamese Preferred: • Experience in community health or integrated care settings, • Knowledge of local, state, and federal healthcare regulations, • Strong analytical, critical-thinking, and communication skills, • Ability to work effectively within a multidisciplinary team This is a rewarding opportunity to practice in a mission-driven setting that values collaboration, cultural competency, and high-quality patient care.
